一部 零細地域住民의 肝炎 B 바이러스 表面抗原 陽性率에 關한 硏究 = A Study on the Prevalence of Hepatitis B Surface Antigen Among Slum Population

Many research workers have reported that the hepatitis B surface antigen is associated with various hepatic disorders such as primary hepatoma, livercirrhosis, and chronic active hepatitis in addition to the well-known fact about the serum hepatitis. Also some epidemiologic studies indicated that the prevalence of hepatitis B surface antigen was higher among the people living in underprivileged conditions than the well-to-do people.
This study was carried out during two summers old 1977 and 1978 in four urban slum areas, Seoul, Busan, Choon Cheon, and Jeon Joo, as a part of health examination survey. The objective of the study was to identify certain epidemiologic characteristics of the hepatitis B surface antigen prevalence with an effort to find out some contribution factors in terms of transmission mechanism. The presence or absence of the antigen was determined by modified micro-Ouchterlony double immuno-diffusion technique, which is known to be the most simple yet relatively sensitive for the detection of the antigen.
The study was done only for 667 preschoolers in Seoul, but general population (all ages) for other three areas; 210 persons for Busan, 297 for Choon Cheon, and 403 for Jeon Joo.
The results obtained and summarized are as followings:
1. The age-adjusted prevalence rate for the total ???????is 5.4% for both sexes, which ??????be higher than that of the rural popualtion. The prevalence rate for males was 4.7% compared with 5.8% for females.
2. Larger city, Busan showed higher prevalence rate 7.0% than smaller cities; Choon Cheon 5.9% and Jeon Ju 4.6%.
3. Age-specific prevalence rate of hepatitis B surface antigen revealed the highest among the young adults, 20-29 years of age in both sexes.
4. Among the preschoolers, the prevalence rate was 6.3% for infants (0-1), 1,9% for toddlers (2-3 years of age), and after this age the prevalence increased.
5. The hepatitis B surface antigen showed a prominant familial aggregation pattern that the house-holds with the antigen positive number had 8.3 times higher positive rate than the average.
6. The tendency of familial aggregation was stronger among children with numerous siblings than those with fewer, and among male siblings than female siblings or sibling of different sex.
7. The analysis of possible contributing factors such as health status, religion, habits of smoking and alcohol drinking, type of water supply, and type of privy showed no association with the positive rate of the hepatitis B surface antigen.
8. Statistical association between the presence of the antigen and ABO blood type was not consistent; there was a strong association between the blood type AB and OO in males only, which needs further study for conclusion.
9. The prevalence rate of Anti-HBs was 1.2%, which increased remarkably after the age of 49 years.
