본 연구의 목적은 40대~50대 중년 여성에게 많이 일어나는 질환인 비특이적 요통에 대해 8주간 모션테이핑을 적용하였을 때 통증, 근활성도, 근경직도 및 긴장도, 기능장애, 균형에 모션테이핑의 효과를 알아보기 위해 실시하였다.
본 연구는 비특이적 요통이 있는 성인 여성 환자 34명을 대상으로 실시하였다. OO시에 위치하고 있는 정형외과 두 곳의 병원에서 실시하였으며, 2022년 7월에서 9월까지 실시하였다.
본 연구는 참여한 대상자들에 대해 연구에 참여하기 전 연구 목적을 충분히 설명하고, 자발적으로 동의를 받았다. ‘임상실험에 대한 권고 사항’ 헬싱키 선언에 따라 진행하였고, 동방문화대학원대학교 기관생명위원회의 승인(8223202206-HR-011-01)을 받았다. 환자에게 요통으로 인한 중재 방법에 대한 목적 및 내용을 설명하고, 중간 포기 의사가 있을 때, 신체 이상 등의 문제가 있을 때에는 즉시 연구 참여 철회를 할 수 있음을 사전에 설명하고 실시하였다.
총 34명의 대상자를 두 집단으로 통계 프로그램인 ‘r’을 이용하여 무작위 방법으로 나누고, 한 집단에는 물리적 인자치료 후 단순 운동치료와 모션테이핑을 함께 적용한 ‘실험군’으로, 다른 한 집단에는 물리적 인자치료 후 단순 운동치료를 적용한 ‘대조군’으로 나누어 실시하였다. 대상자 선정기준은 전문의에 의해 요통 진단을 받은 사람, 40~50대 중년 여성, 통증시상척도(VAS)가 4이상인 사람, 요통 이외에 다른 신경계 질환을 가지지 않은 사람으로 하였다.
실험군과 대조군 모두에 비특이적 요통의 치료적 방법으로 물리치료를 실시하였다. 물리치료는 온열 치료 20분, 간섭전류치료 15분, 초음파 치료 5분을 하였다. 치료를 마치고 난 후 실험군에는 단순 운동치료를 실시함에 있어 모션테이핑을 적용하고, 대조군에는 아무런 처치를 하지 않았다. 치료가 종료된 후에는 테이핑을 그대로 부착하고 생활하였다. 통증의 측정을 위해서 SF-MPQ; short-form McGill Pain Questionnaire를 사용하였다. 척추세움근(erector spinae)의 근활성도를 측정하기 위하여 근전도 장비를 사용하여 측정 하였다. 근경직도 및 근긴장도는 Myoton PRO (Myoton AS, Estonia)장비를 사용하였다. 기능 장애 정도를 평가하기 위해 Oswestry Disability Index를 이용하였고, 균형을 측정하기 위해 Biodex balance system을 사용하였다.
집단 별 통증, 근활성도, 근경직도 및 근긴장도, 기능장애, 균형 요인을 분석하기 위해 중재 전/후의 값을 대응표본 t-검정 방법을 사용하여 비교 분석하였으며 집단 간 차이는 중재 전, 중재 후, 중재 전-후 변화값을 독립표본 t-test를 실시하여 비교 분석하였다. 통계적 유의수준은 0.05로 하였다.
중재에 따른 결과는 다음과 같다. 통증과 관련된 한국형 단축 맥길 통증 척도 중 감정적, 정서적 항목은 실험군, 대조군 모두에서 중재 전-후 비교에서 통계적으로 유의한 차이가 나타났다. 통증 수치와 통증 강도 역시 실험군과 대조군 모두 중재 전-후 비교에서 통계적으로 유의한 차이가 나타났다. 중재에 따른 동적 근활성도 변화는 실험군과 대조군 모두에서 통계적으로 유의한 차이가 나타났다. 앉은 자세에서의 정적 근활성도는 실험군에서는 통계적으로 유의한 감소를 나타내었으나, 대조군은 통계적으로 유의한 차이를 나타내지 않았다. 근경직도와 근긴장도는 실험군과 대조군 모두 중재에 따른 변화에서 통계적으로 유의한 감소를 나타내었다. 기능장애 수치는 실험군과 대조군 모두 중재에 따른 적용에서 통계적으로 유의한 감소를 나타내었다. 정적 균형은 실험군에서 중재에 따른 적용에서 통계적으로 유의한 감소를 나타내었다. 그러나 대조군의 경우 중재에 따른 변화에서 통계적으로 유의한 변화를 나타내지 않았다. 동적 균형은 실험군에서 통계적으로 유의한 감소를 나타내었다. 그러나 대조군의 경우 중재에 따른 변화에서 통계적으로 유의한 변화를 나타내지 않았다.
집단 간 통계적 비교에서 통증의 경우 감정적, 정서적 항목의 수치는 중재 전 두 집단 간 유의한 차이가 나타나지 않았고, 중재 후 실험군과 대조군의 두 집단 간 차이는 통계적으로 유의한 차이를 나타내었다. 통증 수치는 중재 전 두 집단 간 유의한 차이가 나타나지 않았고, 중재 후 실험군과 대조군의 두 집단 간 차이는 통계적으로 유의한 차이를 나타내었다. 통증 수치는 중재 전, 중재 후 모두 두 집단 간 유의한 차이가 나타나지 않았다. 동적 근활성도는 중재 전 두 집단 간 유의한 차이가 나타나지 않았고, 중재 후 실험군과 대조군의 두 집단 간 차이는 통계적으로 유의한 차이를 나타내었다. 정적 근활성도는 중재 전, 중재 후에서 두 집단 간 차이는 통계적으로 유의하게 차이가 나타나지 않았다. 근경직도와 근긴장도는 중재 전 두 집단 간 유의한 차이가 나타나지 않았고, 중재 후 실험군과 대조군의 두 집단 간 차이는 통계적으로 유의한 차이를 나타내었다. 기능장애는 중재 전 두 집단 간 유의한 차이가 나타나지 않았고, 중재 후에는 실험군과 대조군의 두 집단 간 차이는 통계적으로 유의한 차이를 나타내었다. 정적 균형과 동적 균형은 중재 전 두 집단 간 유의한 차이가 나타나지 않았고, 중재 후 실험군과 대조군의 두 집단 간 차이는 통계적으로 유의한 차이를 나타내었다.
비특이적 요통이 있는 중년여성에게 모션테이핑은 통증, 근활성도, 근경직도, 근긴장도, 기능장애 및 균형에 긍정적으로 유의한 변화를 나타내었다. 이는 비특이적 요통이 있는 중년여성에게 모션테이핑이 매우 긍정적인 효과가 있다는 것을 나타낸다.
앞으로 본 연구의 결과를 바탕으로 비특이적 요통이 있는 중년 여성에게 모션테이핑을 효과적인 중재방법으로 적용될 수 있을 것이며, 임상에서 가장 흔한 근골격계 질환에 대해 좋은 중재방법으로 제안할 수 있을 것이다.
The purpose of this study was to find out the effects of motion taping on pain, muscle activity, muscle stiffness and tone, dysfunction, and balance when motion taping was applied for 8 weeks for non-specific low back pain, a disease that occurs frequently in middle-aged women in their 40s and 50s.
This study was conducted on 34 adult female patients with non-specific low back pain. It was conducted at two orthopedic hospitals located in OO city, and was conducted from July to September 2022.
In this study, the purpose of the study was fully explained to the participants before participating in the study, and consent was obtained voluntarily. It was carried out in accordance with the Helsinki Declaration of 'Recommendations for Clinical Trials' and was approved by the IRB of Dongbang Culture Graduate University (8223202206-HR-011-01). The contents and purpose of the intervention method due to back ain were explained to the patient, and it was explained in advance that participation in the study could be withdrawn immediately if there was an intention to give up in the middle or if there were problems such as physical abnormalities.
A total of 34 subjects were randomly divided into two groups using the statistical program 'r', and one group was divided into an 'experimental group' in which simple exercise therapy and motion taping were applied together after physical therapy, and the other group was divided into a 'control group' to which simple exercise therapy was applied after physical therapy. Subject selection criteria were follows. They had been diagnosed with low back pain by a specialist, middle-aged women in their 40s and 50s, those who had a visual analogue scale (VAS) of 4 or higher, and those who had no other neurological disease other than back pain.
Pain management through physical therapy, a non-surgical method, was administered to both the experimental group and the control group as a therapeutic method for non-specific low back pain. Physical therapy was applied for 20 minutes of thermotherapy, 15 minutes of interference current therapy, and 5 minutes of ultrasound therapy. After finishing the treatment, motion taping was applied to the experimental group in performing simple exercise therapy. The short-form McGill Pain Questionnaire was used for pain measurement. In order to measure the muscle activity of the erector spinae, it was measured using electromyography. Myoton PRO (Myoton AS, Estonia) equipment was used for muscle stiffness and muscle tone. The Oswestry Disability Index was used to evaluate the degree of functional impairment, and the Biodex balance system was used to measure balance.
In order to analyze the pain, muscle activity, muscle stiffness and tone, dysfunction, and balance factors by group, the values before and after the intervention were compared and analyzed using the paired-sample t-test method. The change values of pre and post intervention were compared and analyzed by conducting an independent sample t-test. The statistical significance level was set at 0.05.
The results of the intervention are as follows. In the emotional and affective factors of the short-form Korean McGill Pain Scale related to pain, statistically significant differences were found in both the experimental group and the control group pre and post intervention. Pain level and pain intensity also showed statistically significant differences between the experimental group and the control group. There was a statistically significant difference in the dynamic muscle activity change according to the intervention in both the experimental group and the control group. Static muscle activity in the sitting position showed a statistically significant decrease in the experimental group, but no statistically significant difference in the control group. Muscle stiffness and muscle tone showed a statistically significant decrease in changes according to the intervention in both the experimental group and the control group. The functional disability level showed a statistically significant decrease in application according to the intervention in both the experimental group and the control group. Static balance showed a statistically significant decrease in coverage following the intervention in the experimental group. However, in the case of the control group, there was no statistically significant change in the change according to the intervention. Dynamic balance showed a statistically significant decrease in the experimental group. However, in the case of the control group, there was no statistically significant change in the change according to the intervention.
There was no significant difference between the two groups in the values of emotional and affective items pre intervention. Comparison between the two groups of the experimental group and the control group post intervention showed a statistically significant difference. There was no significant difference in pain level before intervention between the two groups. After the intervention, the difference between the two groups, the experimental group and the control group, showed a statistically significant difference. There was no significant difference in pain level between the two groups pre and post intervention. There was no significant difference between the two groups in dynamic muscle activity before intervention. After the intervention, the difference between the two groups, the experimental group and the control group, showed a statistically significant difference. There was no statistically significant difference in static muscle activity between the two groups pre and post intervention. There was no significant difference between the two groups in muscle stiffness and muscle tone before intervention. After the intervention, the difference between the two groups, the experimental group and the control group, showed a statistically significant difference. There was no significant difference in functional disability between the two groups before intervention. After the intervention, the difference between the two groups, the experimental group and the control group, showed a statistically significant difference. There was no significant difference between the two groups in static balance and dynamic balance before intervention. After the intervention, the difference between the two groups, the experimental group and the control group, showed a statistically significant difference.
In middle-aged women with non-specific back pain, motion taping showed positively significant changes in pain, muscle activity, muscle stiffness, muscle tone, dysfunction, and balance. This indicates that motion taping has a very positive effect on middle-aged women with non-specific low back pain.
In the future, based on the results of this study, motion taping can be applied as an effective intervention method for middle-aged women with non-specific low back pain, and it can be suggested as a good intervention method for the most common musculoskeletal disorders in clinical practice.
