노인의 동적평형성에 영향을 미치는 요인 분석 = Analysis of Factors Affecting the Dynamic Balance of the Elderly

The purpose of this study was to examine what factors affected the dynamic balance of elderly people of sixty and over in a bid to lay the foundation for providing motor intervention for the elderly not to get hurt from a fall.
The subjects in this study were 122 elderly people who were 60 years old and up and used a senior club in the city of C. They included41 men and 81 women. They were at the mean age of 69.46±5.03. Their average height and weight were 1.56±0.08cm and 59.55±9.26kg. Sensorimotor tests were conducted since the sensorimotor organ seemed to have an impact on dynamic balance. Their motor-related basic physical fitness was tested by implementing a chair-stand test, two-minute step test, chair sit-and-reach test, 8-foot-up-and-go test and MDRT(multidirectional reach test). Their range of ankle motion was evaluated as well. In terms of sense, M-CTSIB(clinical test of sensory interaction in balance) was employed, and age and BMI were selected as additional factors that might affect dynamic balance.
For data handling, independent-samples t-test was used to see if there were any differences between the men and women in dynamic balance, and step wise multiple regression analysis was utilized toidentify which factors affected dynamic balance.
The findings of the study were as follows;
1. When independent samples t-test was employed to look for any differences in dynamic balance by gender, there appeared no significant difference between the elderly men and women in left/right dynamic balance, but a significant gap was found in front/rear dynamic balance(p<0.05).
2. The elderly men were older than the elderly women, and the former excelled the latter in muscular strength of the lower part of the body, aerobic endurance, front stability, rear stability, right stability and left stability. The former got better scores in M-CTSIB2 and 3 than the latter. The latter surpassed the former in BMI, agility, flexibility of the lower part of the body, range of ankle motion and M-CTSIB4.
There were differences between the collective averages of the two, but the gaps were not significant. But significant differences were found in age, flexibility of the lower part of the body, range of ankle motion and rear stability(p<0.05).
3. When multiple regression analysis was carried out to find out the impact of the factors on left/right dynamic balance, the elderly people who had a better sensory information literacy, who had a better muscular strength in the lower part of the body and who were less older were better at left/right balance(p<0.05).
4. As a result of making multiple regression analysis to check into which factors affected the front/rear dynamic balance of the elderly people, the elderly people who had a better sensory information literacy, who were less older and whose BMI was lower were better at front/rear dynamic balance(p<0.05).
5. When multiple regression analysis was employed to grasp the influence of the factors on the elderly men's left/right dynamic balance, those who were better agile and whose range of ankle motion was broader were better at left/right dynamic balance(p<0.05).
6. When multiple regression analysis was conducted to figure out the impact of the factors on the elderly men's front/rear dynamic balance, those whose sensory information literacy was better, whose front stability was better and who were less older were better at front/rear dynamic balance(p<0.05).
7. As a result of implementing multiple regression analysis to find out the impact of the factors on the front/rear dynamic balance of the elderly women, those who had a better sensory information literacy and whose muscular strength was better in the lower part of the body were better at front/rear dynamic balance(p<0.05).
8. When multiple regression analysis was carried out to see how the factors affected the front/rear dynamic balance of the elderly women, those who had a more excellent sensory information literacy and whose BMI was lower were better at front/rear dynamic balance(p<0.05).
The above-mentioned findings suggested that the elderly people who were less older, who had a more excellent sensory information literacy, who had a stronger muscular strength in the lower part of the body and whose BMI was lower were more excellent in terms of dynamic balance. The elderly people were deteriorated in every regard with age, and the sensory organ had a huge impact on dynamic balance. To produce the best results, programs that provides motor intervention for elderly people not to be injured from a fall should cater to less older people, and a wide variety of sustained exercise programs that consist of sensory exercise, muscular workout and flexibility workout should be provided. Elderly people should be encouraged to keep moving their bodies to lower their BMI, and nutrition education should be offered to change their dietary habits(p<0.05).
이 연구의 목적은 노인들의 낙상예방을 위한 운동중재의 기초자료를 제공하고자 60세 이상의 노인을 대상으로 동적평형성에 영향을 미치는 요인을 분석하고자 하였다.
피험자는 C시 시니어클럽을 이용하는 60세 이상 노인 122명(남성:41명, 여성:81명)으로 연령은 69.46±5.03세이며, 평균 신장은 1.56±0.08cm이었으며, 평균 체중은 59.55±9.26이었다. 측정은 동적평형성에 영향을 미칠 것으로 예상되는 운동계, 감각계 검사를 실시하였다. 운동계와 관련된 기초체력검사는 하체근력(Chair Stand Test), 2분 걷기(2-Minute Step Test), 하체유연성(Chair Sit - And Reach Test), 민첩성(8-Foot Up And - Go Test), 발목가동범위, MDRT(Multidirectional Reach Test)를 측정하였고, 감각계 검사로는 M-CTSIB(Modified version of the Clinical Test of Sensory Interaction Balance)를 측정하였다. 또한 연령 및 BMI를 예상요인으로 부가하여 분석하였다.
자료처리는 남, 여 동적평형성의 차이를 보기 위해 독립 t-검정을 사용하였으며, 동적평형성에 영향을 미치는 요인을 분석하기 위해 중다 회귀분석(stepwise)을 사용하였다.
1. 노인의 남, 여 성별에 따라 좌/우 동적평형성은 유의한 차이를 보이지 않았으나 전/후 동적평형성은 유의한 차이를 보였다(p<.05). 남성이 여성에 비해 전/후 동적평형성 이 낮다.
2. 노인의 남, 여 성별에 따른 동적평형성 예측 요인 간의 차이를 살펴본 결과남성노인은 연령, 하체근력, 유산소지구력, 전면, 후면, 우측, 좌측 안정성, M-CTSIB 2,3의 평균이 여성노인에 비해 높았으며, 여성노인은 BMI, 민첩성, 하체유연성, 발목가동범위, M-CTSIB 4의 평균이 남성노인에 비해 높게 나타났다. 평균의 차이는 있었으나 유의한 차이를 보이지 않았으며 연령, 하체 유연성, 발목가동범위, 후면안정성은 유의한 차이를 보였다(p<.05).
3. 노인의 좌/우 동적평형성에는 M-CTSTB4, 연령, 하체근력 순으로 영향을 미치는 것으로 나타났다. 감각정보사용능력, 하체근력이 좋을수록, 연령이 낮을수록 좌/우 동적평형성의 수준이 높았다(p<.05).
4. 노인의 전/후 동적평형성에는 연령, M-CTSIB4, BMI 순으로 영향을 미치는 것으로 나타났다. 감각정보사용능력이 좋을수록, 연령과 BMI는 낮을수록 좌/우 동적평형성의 수준이 높았다(p<.05).
5. 남성 노인의 좌/우 동적평형성에는 민첩성, 발목가동범위 순으로 영향을 미치는 것으로 나타났다. 민첩성과 발목가동범위가 좋을수록 남성 노인의 좌/우 동적 평형성의 수준이 높았다(p<.05).
6. 남성 노인의 전/후 동적평형성에는 연령, 전면안정성, M-CTSIB4 순으로 영향을 미치는 것으로 나타났다. 연령이 낮을수록, 감각정보사용능력과 전면안전성이 좋을수록, 남성 노인의 전/후 동적평형성의 수준이 높았다(P<.05).
7. 여성 노인의 좌/우 동적평형성에는 M-CTSIB3, 하체근력 순으로 영향을 미치는 것으로 나타났다. 감각정보사용능력과 하체근력이 좋을수록, 여성 노인의 좌/우 동적평형성의 수준이 높았다(p<.05).
8. 여성 노인의 전/후 동적평형성에는 BMI, M-CTSIB3, M-CTSIB4 순으로 영향을 미치는 것으로 나타났다. 신체질량지수가 낮을수록, 감각정보사용능력이 좋을수록, 여성 노인의 좌/우 동적평형성의 수준이 높았다(p<.05).
이와 같은 연구 결과로 볼 때 노인의 좌/우, 전/후 동적평형성은 연령이 낮을수록, 감각계정보사용능력과 하체근력이 좋을수록, BMI가 낮을수록 동적평형성의 능력이 높게 나타났다. 연령이 증가 할수록 모든 기능이 감소함을 볼 수 있었으며 감각계가 동적평형성에 많은 영향을 미치는 것을 볼 수 있었다. 따라서 낙상예방프로그램 운동 중재에 있어서 연령의 기준을 낮춰서 실시하여야 효과를 극대화 할 수 있으며 감각향상운동, 근력운동, 유연성 운동을 포함한 다양하고 지속적인 운동프로그램이 운영되어야 하고, 신체질량지수를 낮추기 위한 신체활동량의 증가 및 식습관 변화를 위한 영양교육이 실시되어야 한다고 생각된다.
