네팔 지진 생존자들의 피해경험 실태와 심리치료 프로그램 효과성 연구 = Research on the Survivors of Nepal Earthquake Analyzing Damage Suffered and Effectiveness of the Psychotherapy Program

본 연구의 목적은 네팔 지진 생존자들의 피해경험을 파악하고 부모자녀 상호작용 증진 개입프로그램(Filial Therapy)의 효과성 검증을 통해 지진 생존자를 위한 효과적인 지원방안을 모색하는 것이다. 이를 위해 네팔 지진피해 이후 텐트촌에 거주하는 부모와 아동 200쌍을 대상으로 설문조사 및 척도검사를 통해 생활실태와 욕구를 비롯한 심리사회적 경험과 지진 전후의 변화, 심리적 스트레스와 신체화 증상에 영향을 미치는 요인을 파악하였다. 또한 질적 사례연구를 통해 지진 전후 변화와 경험을 분석하고, 부모자녀 상호작용 및 관계증진을 위한 프로그램을 실시하고 효과성을 검증하였다.
연구결과는 다음과 같다. 첫째, 설문조사를 통해 일반적 현황, 지진에 의한 삶의 변화, 사회적 지지를 포함한 생활실태와 욕구를 분석하였다.
분석결과, 일반적 현황에서 조사응답자 부모는 대부분이 여성이었고 문맹율이 높았다. 직업 유무에 있어 다수 무직으로 직업이 있는 경우에도 청소직이나 수공업 등에 편중되어 안정적이지 못한 수입을 갖고 있었다. 이는 네팔의 낙후한 경제적 상황으로 인해 무직자가 많은 것도 원인이라고 할 수 있다. 또한 월수입은 평균 55,000루피, 한화로 약 6만원으로 평균 수입에 못 미치는 조사응답자가 과반수를 넘었다. 이는 네팔의 최저생계비에 못 미치는 금액이라고 할 수 있다.
지진에 의한 삶의 변화 분석결과, 대부분의 조사응답자가 1년 이상 텐트촌에 거주한 것으로 나타났으며, 건강상태는 부모는 건강하지 않다고 느끼는데 반해 아동은 건강하다고 느끼고 있는 것으로 나타났다. 또한 지진으로 인한 질병 발병 여부, 가족구성원의 변화, 부상·사망한 가족구성원 유무, 직업변화는 큰 차이를 보이지 않았으나, 집안 신상 존재와 하루기도 횟수에서 변화가 있는 것으로 나타났다. 이러한 결과는 종교의 영향력이 강한 네팔의 문화적 특성에서 기인하며, 지진 이후 이와 같은 변화가 두드러진 것을 확인할 수 있다.
사회적지지 분석결과, 삶의 만족도는 부모와 아동 모두 자신의 삶에 만족한다고 응답한 비율이 더 높았으며, 가까운 친구와 이웃이 있는 비율이 더 높았다. 또한 부모와 아동의 절반 이상이 지역 내 회합장소가 있는 것으로 조사되었으며, 종교의존도가 높은 것으로 나타났다. 이러한 결과를 통해 조사응답자의 높은 삶의 만족도를 확인할 수 있었으며, 특히 가까운 이웃이나 친구, 회합장소를 통해 사회적 관계를 맺고 있는 것으로 나타났다. 즉 사회적 시스템의 결핍을 이러한 인적 자원을 통해 해결해 가는 것으로 판단된다. 또한 부모와 아동 모두 높은 종교 의존도를 보여 종교의 영향력이 강한 네팔의 문화적 특성을 다시 한 번 확인할 수 있었다.
이어서 생활실태와 욕구를 분석한 결과를 요약하면 다음과 같다. 먼저 필수 자원에의 접근을 조사한 결과, 식사횟수는 하루 평균 두 끼를 섭취하고 있었으며, 식수접근성과 관련해 식수와 생활용수를 공동으로 사용하는 비율이 높게 나타났다. 식수와 생활용수는 주로 공공수도나 물탱크를 통해 얻고 있는 것으로 나타나 텐트 촌 내 깨끗한 물의 충분한 공급과 더불어 식수위생에 대한 교육이 이루어져야함을 논하였다. 아동보호실태 조사결과, 부모와 아동 대다수가 범죄 또는 폭력에의 노출경험이 없고 안전에 대한 걱정이 없는 것으로 나타났으나, 위험대처방법에 대해서는 대다수가 모르고 있는 것으로 나타났다. 건강실태 조사결과, 질병 발생 시 병원을 주로 이용하고 있는 것으로 나타났으며 참는다는 응답도 비교적 높았다. 또한 1회 진료시 의료비용을 일부 내고 있었으며, 최근 2개월간 의료기관 이용 경험이 없는 비율이 더 높았다. 교육실태와 관련해서는 도보 1시간 이내 학교가 있다고 응답한 비율이 높고 학교까지의 소요시간도 도보 30분 이내가 가장 높게 나타나 교육 접근성은 상대적으로 좋은 편인 것으로 조사되었다. 아동의 학교 출석 횟수는 `주 5회`가 가장 높은 비중을 차지했으며, 교과서를 가지고 있는 아동의 비중이 높았다. 이어서 방과후 활동 종류는 집안일과 학습을 꼽아 방과후 놀이 보다는 주로 집안일 또는 학습을 하고 있는 것으로 나타났다. 마지막으로 지진 이후 심리치료 또는 심리정서 프로그램과 같은 심리지원 서비스 이용경험이 높게 나타났으며, 이용의사와 관련해 조사 대상자의 70.9%가 이용의사가 있는 것으로 나타나 이러한 주민의 욕구를 반영해 심리·정서적 어려움을 겪는 지진 생존자를 대상으로 심리지원을 위한 치료 프로그램 등 서비스가 마련되어야 함을 시사하였다.
각 주요 영역별 욕구를 분석한 결과 주거영역은 지진으로부터 안전한 공간 제공(주거), 더 많은 교육시간(교육), 충분한 의료서비스 이용(건강), 깨끗한 식수공급(식수위생), 안전한 주거환경 마련(아동보호)을 각 영역별 우선순위로 꼽았다. 이를 통해 지진 피해 이후 심리·정서적 어려움을 겪는 지진 생존자를 대상으로 심리지원을 위한 치료 프로그램의 필요성과 더불어 각 영역별 나타난 욕구의 우선순위를 근거로 주민의 욕구를 반영한 지원 및 개입이 필요함을 제시하였다. 특히 네팔 정부의 주민의 욕구를 근거로 현 임시거주지에 대한 관리 (Camp Management), 단계별 지원계획수립, NGO/INGO 단체들의 원활한 사업 수행을 위한 규제완화 및 협력의 필요성을 제시하였다.
둘째, 설문조사 및 척도조사를 통한 부모와 아동의 심리적 스트레스 및 신체화 증상을 조사한 결과 높은 심리적 스트레스와 신체화 증상을 보이는 것으로 나타났다. 구체적으로 부모의 절반 이상이 사건충격은 임상적 위험군, 성인행동문제는 고위험군에 속하는 것으로 나타났으며, 신체화 증상은 위험군에 속하는 것으로 나타났다. 아동은 42%가 외상증상을 갖고 있었고 아동의 행동문제에서 위험군에 속하는 비율이 높았다. 또한 신체화 증상은 위험군에 속하는 것으로 나타났다. 이어서 부모의 심리적 스트레스 및 신체화 증상에 미치는 영향요인을 살펴보면, 먼저 부모의 성인행동문제는 직업, 건강상태, 지진으로 인한 질병 발병, 지진 후 가족구성원의 변화, 삶의 만족도, 종교 의존도에 따라 유의미한 차이가 있었다. 즉 직업이 있고 건강상태가 좋지 않고, 지진으로 인해 질병이 있고 가족구성원 변화가 있는 집단에서 행동문제가 높게 나타났으며, 삶의 만족도와 종교 의존도가 낮은 집단이 행동문제를 더 많이 보고했다. 신체화 증상은 무학이고 건강상태가 좋지 않은 집단, 지진 전 질병이 있었고 지진으로 인해 질병이 발병한 집단, 삶의 만족도가 낮고 긴급구조 서비스를 요청할 수 있다는 것을 인식하지 못하는 집단이 신체화 증상이 더 높게 나타났다. 아동의 행동문제는 12세 미만이고 지진으로 하루기도 횟수가 줄었다고 응답한 집단, 종교의존도가 높다고 응답한 집단의 부모들이 아동 행동문제가 높은 것으로 나타났으며, 여자이고 건강상태가 좋지 않으며 지진 전 질병이 있었던 집단, 하루기도 횟수가 줄고 비상시 긴급구조서비스를 요청할 수 있다는 인식과 회합 장소 인식이 없는 집단에서 신체화 증상을 더 많이 보고했다. 아동의 신체화 증상은 여자이고 건강상태가 좋지 않으며 지진 전 질병이 있었던 집단, 하루기도 횟수가 줄고 비상시 긴급구조서비스를 요청할 수 있다는 인식이 없는 집단, 회합 장소 인식이 없는 아동집단이 신체화 증상을 더 많이 보고했다. 특히 종교적 영향력을 고려하지 않은 선행연구와는 달리 본 연구에서는 종교의 존도, 하루기도 횟수 변화 등 종교문화적 요인이 심리적 스트레스와 신체화 증상에 영향을 미치는 주요한 요인임을 밝혔다.
셋째, 질적 사례연구를 통해 아동, 부모, 관계자들의 지진 전/후의 변화와 경험을 분석하였다. 분석결과, 지진은 물리적 환경이 무너지고 생명을 앗아갈 뿐만 아니라 남아있는 생존자들의 삶도 함께 무너뜨린 것으로 나타났다. 지진에 따른 심리적 충격은 무기력함을 가져왔는데, 특히 가정을 이끌어 가는 아버지에게 두드러지게 나타났으며, 이는 희망에 대한 상실감도 수반하였다. 다음으로 부모와 아동 모두 지진 전은 다시 돌아갈 수 없는 시절로 기억하고 있었는데 아동들은 친구와 신나게 놀고 떠들던 그 시절로 돌아가고 싶지만 이제는 걱정과 두려움, 무서움을 모른척하며 살 수 없게 되었다. 부모는 지진으로 생계를 이어 가던 일자리도 사라지고, 할 일도 없고, 할 힘도 없어져 지진 이전의 삶은 복구가 불가능한 시절로 남았다. 이어서 생존자들의 삶은 지진 당시 두려움과 공포가 반복되는 심리적 고립상태에 있는 것으로 나타났다. 정부가 제 역할을 하지 못하고 보상 약속을 지키지 않는 모습, 자원배분에 있어서 불공평한 분배, 국제사회에서 잊혀져가는 현실로 인해 또 다른 상실감을 경험하고 있었다. 마지막으로 지진이 남긴 일상이 된 두려움은 삶의 한부분이 되어 공존하고 있었으나, 현 상황에서 생존자들은 다시 일어설 수 있기를 바라며 소망하고 있었다. 학교는 아이들이 네팔의 미래를 꿈꾸게 하는 산실이 되어 있었고, 부모는 교육을 통해 아이들과 자신이 좀 더 나은 삶을 살 수 있을 것이라는 믿음을 가지고 있었다. 이러한 결과를 통해 국제 비영리기관(INGO) 지원에 대한 네팔정부의 체계적인 관리와 행정적 지원, 네팔정부의 계획적이고 투명한 자원 배분, 주거지 마련 및 이주계획 수립, 아동 심리정서지원사업의 체계적 실시, 인프라 구축과 더불어 분야별 현지 인력 양성 등을 제언하였다.
마지막으로 부모자녀 상호작용 증진 개입프로그램을 실시하고 프로그램의 효과를 분석하였다. 분석결과, 부모자녀 상호작용 증진 개입프로그램은 네팔 지진 생존 부모의 지진 사건에 대한 충격을 유의미하게 감소시켰으며, 부모의 성인행동문제의 하위 영역 중 사고 문제를 제외한 불안/우울, 위축, 신체적 증상, 주의집중문제, 공격 행동, 규칙위반, 내재화문제 및 외현화문제, 성인행동문제에서 모두 유의미한 개선이 있었다. 또한 부모의 신체화 증상 역시 유의하게 감소되었다. 아동의 경우에도 아동행동문제 모든 하위 영역 즉 불안/우울, 위축/우울, 신체적 증상, 사회적 미성숙, 사고 문제, 주의집중문제, 공격행동, 규칙위반, 내재화문제 및 외현화문제에서 유의미한 개선이 있었다. 이러한 결과는 부모-자녀 간의 관계개선과 의사소통을 통한 놀이치료 프로그램이 부모의 지진 사건충격 정도, 성인행동문제 및 신체화 증상 그리고 아동의 행동문제를 완화시킬 수 있음을 보여주었다.
이상의 연구결과를 통해 네팔 정부는 지진 생존자들의 실태와 욕구를 고려해 각 국제 비영리기관(INGO) 의 전문성을 고려한 활동 지역 배치 및 역할분담 등 복구사업에 대해 체계적인 지원 방안을 마련해야 함을 보여주고 있다. 특히 국제 NGO의 사업활동과 지역을 확대해 효과적인 복구와 재건이 전개될 수 있도록 해야 한다. 또한 텐트촌 주민의 안전한 주거환경 확보를 위한 단계별 지원 정책, 열악한 텐트촌 환경에 대한 텐트촌 관리(Camp Management) 강화, 비상시 긴급구조서비스 체계와 아동 안전교육제도 강화 등 텐트촌에 거주하는 부모와 아동이 안전하게 보호 받을 수 있도록 구체적이고 실제적인 대안 마련이 요구된다. 반면 실천적 측면에서는 지진 이후 생존자들이 경험하고 있는 심리적 스트레스와 신체화 증상 완화를 위한 심리정서 지원방안을 마련해야 한다. 지진 발생 후 1년이 지난 시점에도 부모와 아동 대다수가 심리적 스트레스와 신체화 증상을 호소하고 있는 것으로 나타나, 네팔의 종교문화적 특성을 고려한 심리정서지원 프로그램의 마련, 정부와 NGO, 대학 간 협조를 통한 현지 전문가 양성, 이를 위한 국제 NGO의 인력 및 재정지원이 필요하다. 더불어 국제 NGO는 실태조사 및 욕구조사를 토대로 한 사업 전개, 지진 발생 시를 대비한 대처방안과 현지 교육 프로그램에 안전교육을 포함하고 재난현장에 맞는 실천적 콘텐츠 및 평가도구 개발이 이루어져야 한다.
본 연구는 텐트촌에 거주하고 있는 재난 생존자, 특히 재난 취약계층인 아동을 포함한 부모와 아동을 대상으로 실태 및 욕구조사를 수행하였다는 점, 그동안 재난 이후 물적 피해에 대한 복구나 지원에 집중해온 한계를 넘어 다른 영역에 비해 상대적으로 미흡했던 심리정서적 부분에 초점을 맞추었다는 점, 다양한 대상과 문화권에서 이미 그 효과가 입증된 프로그램인 부모자녀 상호작용 증진 개입 프로그램 실시를 통해 네팔에서의 효과성을 검증하였다는 데 의의가 있다. 이를 통해 지진과 같은 자연재난으로 인해 어려움을 겪는 국가에 대한 효과적인 지원방안을 모색하는 데 필요한 기초자료로 활용할 수 있을 것이다.
The aim of this research is to examine the experiences of the Nepal earthquake survivors and the effectiveness of the Filial Therapy intervention program and propose more effective measures to aid the survivors of the earthquake. We used a questionnaire and a quantitative survey using a scale of 200 parents and their children, who have been living in the tent city since the Nepal earthquake disaster, to assess the actual living conditions and needs of the earthquake victims, their psychosocial experiences, changes that had happened since the earthquake, and also the factors that were contributing to psychological stress and somatic symptoms. We also carried out qualitative case studies in order to analyze the changes that people encountered after the earthquake and survivors` experiences, and investigate the effectiveness of the programs that had been implemented to improve interactions and relations between parents and their children.
The results are as follows. Firstly, the questionnaire helped us to analyze the current state of affairs, how life changed because of the earthquake, actual living conditions, including social support systems, and survivors` needs.
The results of our analysis are that of the parents that participated in our questionnaires, the majority of the respondents were female and a high percentage of them were illiterate. In response to the question of whether they were employed or not, most respondents replied that they were unemployed, and where they were employed, an extremely large proportion of respondents were employed in the cleaning or manual industries and thus did not have a reliable source of income. Nepal`s underdeveloped economy may also be considered to be the cause of high rates of unemployment. More than half of the respondents answered that they were earning less than the average monthly income of 55,000 rupee, i.e. 60,000 won, thus they were not earning the minimum income required to meet their living costs.
The results of our analysis of the change in lifestyle due to the earthquake showed that the majority of the respondents had been living in the tent city for more than a year, and the adults felt that they were not in a good state of health while children felt that they were healthy. Responses to questions about the onset of illness and changes in the family unit due to the earthquake, whether family members had been wounded or killed in the earthquake, and changes in employment showed that there were no great changes, but the survey showed that there had been changes in the family religion and also in the number of times that people prayed each day. We were able to confirm these marked changes following the earthquake and attribute these results to the unique nature of the Nepalese culture, in which religion has a strong influence.
The results of the analysis of social support structures are as follows. There were more parents and children who were satisfied with their lives than those who were not, and more respondents who said that they had a close neighbor or friend than those who did not. We also found that more than half of parents and children had access to a meeting place within the region that they were living in, and that they relied more heavily on religion. These results showed us that the respondents were highly satisfied with their lives, and that respondents engaged in social interactions with close friends or neighbors or at places of meeting. Thus we concluded that the lack of a social system was being resolved by means of human resources. Also, the fact that both parents and children relied heavily on their faith again confirmed the unique nature of Nepal`s society that is strongly influenced by religion.
Further, the summary of the analysis of the actual living conditions and needs of the earthquake victims is as follows. First, the results of the survey on access to essential resources are as follows, respondents consume 2 meals a day on average, and a large percent of them used community supplies of potable water and water for everyday needs. Potable water and water for everyday needs were mainly accessed from public water supplies and water tanks. We recommend that enough clean water to be supplied to the tent city and also that education about potable water sanitation be provided. The results of the survey about the actual state of child protection revealed that the majority of parents and children had not been exposed to crime or violence and were not concerned about safety, but the majority of respondents did not know how to handle dangerous situations. The results of the survey on the questions on survivors` actual state of health showed that people were usually using the hospitals when they fell sick but a large proportion of people answered that they merely endured through their illness. Also, people were paying for part of their treatment the first time they had treatment but most people responded that they had not used the hospital in the last two months. In response to questions on the actual state of education, a high proportion of respondents said that there was a school within one hour`s walking distance, the majority of respondents answered that there was a school within 30 minutes walking distance, thus we confirmed that access to education is relatively good. A high proportion of respondents stated that their children attended school 5 times a week, and a large proportion of children also owned text books. In addition, after school activities mainly included household chores and study. Children mainly did household chores or studied after school, rather than spending time playing. Lastly, after the earthquake a high proportion of the respondents underwent psychological treatment and/ or used psychological support services, such as programs helping people with psychological and emotional difficulties. In response to the question on their intention to utilize these services, 70.9% of respondents answered that they intended to use these services, thus in light of the needs of the citizens of Nepal, we suggest that programs and services offering psychological support to the survivors of the earthquake who are experiencing psychological and emotional difficulties need to be made available.
These are the results of our analysis of the requirements survivors had for each important area of life, in terms of residents` requirements the most important priorities for each area are as follows: providing safe areas (shelter), increasing hours of education (education), ample utilization of medical services (health), provision of clean drinking water (water and sanitation), provision of a safe living area and environment (child protection). On the basis of these results, we propose that a treatment program that will offer psychological support to the survivors of the earthquakes who are suffering mentally and psychologically, and intervention and support that reflect the needs of survivors and are based on the priorities outlined in each of the above areas are vital. In particular, we suggest that management of the current temporary shelters (Camp Management) by the government of Nepal, taking the needs of the citizens into account, the government establishing plans to support the victims in each area, and deregulation and cooperation to ensure that NGOs/ INGOs can engage easily in projects are essential.
Secondly, the questionnaire and quantitative survey using a scale investigating the levels of psychological stress that parents and children were experiencing and also symptoms of somatization found that both parents and children were experiencing high levels of psychological stress and also psychosomatic symptoms. More concretely, more than half of parents were suffering from levels of shock caused by the earthquake that classified them as being clinically at risk, and many of them displayed adult behavioral problems that placed them in a high-risk group; more than half of parents were also classed as being at risk in terms of symptoms of somatization. 42% of children showed symptoms of trauma, and a high proportion of them were in the high-risk group in terms of child behavioral problems, they also fell into the risk group in terms of symptoms of somatization. In addition, when we examined the factors involved in parents` psychological stress levels and symptoms of somatization, there was a significant difference in the level of parents` adult behavioral problems, depending on whether they were employed, the state of their health, diseases they suffered as a result of the earthquake, changes to their family unit as a result of the earthquake, their degree of life satisfaction and the degree to which they relied on religion. In short, those in the group who were employed, in poor health, who had fallen sick because of the earthquake, and whose family units had changed displayed high rates of behavioral problems; those in the group who were not satisfied with life and who did not rely on religion displayed even more behavioral problems. The group who had symptoms of somatization, were undereducated and in poor health, the group who were ill before the earthquake, or who had fallen ill as a result of the earthquake, the group who reported low levels of life satisfaction and who did not realize that they were able to request help from the emergency services, displayed higher rates of symptoms of somatization. In terms of children with behavioral problems, the group of children below the age of 12 who prayed fewer times per day after the earthquake, and the group of children whose parents answered that they were highly religious, showed high rates of behavioral problems, the group of girls who were in poor health and had been ill before the earthquake, and the group who prayed less throughout the day, and who did not recognize that they could access emergency services or community meeting places, also reported more symptoms of somatization. In terms of children`s symptoms of somatization, the group who were female, in poor health, who had been sick prior to the earthquake, the group who prayed less throughout the day, and who did not recognize that they were able to request access to emergency services, and the group of children who did not realize that they could access community meeting places reported more symptoms of somatization. Unlike preceding studies which did not take the effects of religion into consideration, our study revealed that dependence on religion, the change in the number of times respondents prayed each day and other elements of the religious culture were important factors influencing psychological stress and symptoms of somatization.
Thirdly, using qualitative case studies, we were able to analyze the experiences and changes that parents, children and other parties connected to the family unit experienced before and after the earthquake. The results of the analysis show that it was not just the physical environment that was destroyed and the lives of the people around them that were snatched away, the lives of the survivors were also destroyed. The psychological stress that accompanied the earthquake brought apathy, which was especially prevalent in the fathers who were heads of the household, and this was accompanied by a loss of hope. Next, both parents and children remembered the times before the earthquake, times that they could never return to, and although children wanted to return to the times when they played excitedly with friends, now they were filled with worry, fear and terror instead. Parents had lost their pre-earthquake livelihoods, they did not have anything to do, or the strength to do anything, and now it is impossible to make things return to the way that it was before the earthquake. Moreover, at the time of the earthquake the survivors felt recurring fear and terror and this caused them to feel psychologically isolated in their lives, survivors still continue to experience these same feelings now. The fact that the government was unable to play its role properly and had not fulfilled its promises regarding compensation, that resources were being distributed but unfairly and that survivors also felt that they were being forgotten by the international society caused them to experience further feelings of loss. Finally, the fear that they experienced due to the earthquake remains with them and is now an everyday part of their lives, however the survivors hope that they will be able to recover from the current situation. Schools are becoming the cradles where children are given dreams for the future of Nepal, parents also believe that education can bring about a better life for them and their children. In light of these results, we suggest that the Nepalese government creates a way to systematically manage the support that international non-profit organizations (INGOs) give, and also provide administrative support, distribute resources in a way that is transparent and strategic, establish plans to provide shelter for the survivors and resettle them, methodically implement services that will support children both psychologically and emotionally, develop infrastructure and simultaneously develop local manpower to manage each of these areas.
Lastly, we implemented programs to improve interactions between parents and children and analyzed their effectiveness. The results of our analysis show that the program to improve interactions between parents and children significantly reduced the shock that parents who had survived the earthquake had suffered as a result of the earthquake, and that with the exception of mental problems, adult behavioral problems i.e. anxiety/depression, feelings of being daunted, somatic symptoms, attention problems, aggressive behavior, tendencies to violate regulations, internalized problems and externalized problems were all improved significantly. Furthermore, the incidence of somatic symptoms in parents was also considerably reduced. In the case of children too, all subcategories of children`s behavioral problems: anxiety/ depression, feelings of being daunted/depression, somatic symptoms, being socially immature, mental problems, attention problems, aggressive behavior, violating regulations, internalized problems and externalized problems were also significantly improved. These results illustrate the fact that programs that improved the relationship between parents and children, and programs where parents and children communicate through play therapy, can help to alleviate the level of shock that parents experienced due to the earthquake, adult behavioral problems, somatic symptoms and also children`s behavioral problems.
The above results showed that the Nepalese government should create systematic support strategies for the rehabilitation project like deploying international non-profit organizations (INGOs) to different regions and allocating roles to them that are based on their expertise while also taking into consideration the actual living conditions and demands of the earthquake survivors. In particular, the government must allow international NGOs to expand both the scope of their projects and the target region for their projects so that effective reconstruction and rehabilitation can take place. Further, in order to ensure the safety of children and parents dwelling in the tent city, we request the provision of practical and concrete alternatives in terms of: support policies for each area that will help to procure a safe shelter for the residents of the tent city, consolidation of the camp management of the substandard tent city environment, and reinforcement of the emergency rescue services system and safety education for children, etc. On the other hand, practical measures to support the survivors psychologically and emotionally in order to reduce the psychological stress and somatic symptoms that the victims are experiencing after the earthquake are also required. One year after the earthquake, the majority of both parents and children are reporting psychological stress and somatization. It is essential that a psychological and emotional support program that takes the unique religious culture of Nepal into account be created, that the government, NGOs and universities cooperate with each other to raise local experts, and that international NGOs provide manpower and financial support. Furthermore, international NGOs must consider surveys of the actual conditions and needs of survivors and develop projects, prepare practical instructional content including measures on how to cope when an earthquake hits and a local education program, which includes safety education, for people affected by disasters, and tools to assess the situation.
This research is significant because of the fact that we surveyed the needs of and the actual circumstances of the survivors of the natural disaster who were living in the tent city, including children who are part of a vulnerable social group, and their parents; we went further than previous research that focused on material loss and how to restore or offer support in material terms, and focused on psychological and emotional difficulties, an area where prior research has been comparatively lacking; further we examined the implementation of a program for the improvement of the interaction between parents and children that has been found effective in various subjects and cultures, and showed that it was also effective in Nepal. This research can be used as essential baseline data when an effective policy of support is being sought for nations that are experiencing difficulties because of earthquakes or other natural disasters.
