Taking each day as it comes & looking at the world through the other's window : a phenomenological study = 만성 정신지환 환자 가족의 간호 체험에 관한 현상학적 연구

본 논문은 만성 정신질환을 않고 있는 환자를 간호하는 가족들의 체험에 관한 실제연구를 발표했으며, 그 가족 간호 자들의 간호 제공에 관한 개개 인적 이야기 진술에 기초되었다. 1960 연대 말과 1970년대의 정신질환자들에 대한 탈 수용화 운동이 촉진된 이래 지역사회 중심의 간호가 강조되어져 왔으며, 그 결과 이러한 환자들의 간호에 대한 가족들의 책임이 현저하게 증가되었다. 이러한 점에서, 임시간호제(respite care service)는 간호인들의 장기 간호 역할로 오는 체력 소모를 감소시키고 그 들의 신체적 정신적 건강을 유지하는데 중요
한 역할을 해 왔다고 할 수 있다. 그럼에도 불구하고 만성 정신질환 자의 가족 간호 자들과 임시간호제에 대한 그들의 인식에 관련된 연구는 미약했다. 그러므로, 묘사적 해석적 연구 방법을 도입한 본 연구의 목적은 이러한 가족 간호 자 개개인의 간호 체험을 가족이라는 상황에서 뿐만 아니라 건강관리와 지역 사회 보조 서비스들과의 좀 더 광범위한 체계상황에서 상세하게 그려 내기 위함이다.
당연시되어지고 간과되어지기 쉬운 가족 간호 행위의 경험들을 연간의 일상적 상황 내에서 조사하기 위해, 연구 방법으로서는 Van Manen 의 해석적 현상학적 접근법을 이용하였으며, 이것은 연구자가 참여자들의 경험들을 밀도 있게 이해하고 만성 정신질환이라는 상황 내에서 가족 간호 제공의 현상을 기술하는데 도움을 주었다. 정신질환을 가지고 있는 가족을 간호하기 위해 지속적으로 전념하여 간호해야 함과 관련 지어 그 가족간호 자들에게 있어서 간호의 의미가 무엇인지를 발견하기 위해, 본 연구는 특히 부모들과 딸로 구성된 14명의 가족 간호자들의 관점에 비추어 그들의 간호 제공 경험과 임시 간호제에 관한 견해에 중점을 두었다. 두차례에 걸친 개별적 심층 면담을 통해 14명의 참여자들은 각각, 그들에게 있어서 만성 정신질환을 않고 있는 사람을위해 간호하는 가족으로서의 삶과 그 간호 행위가 어떤 의미를 지니는지 질문 되어겼다. 기술된 면담 자료들은 그들의 체험의 의미를 주제별로 기술하고 해석함으로써 분석되어졌다.
본 연구를 통해 두 가지의 본질적 주제가 추출되었으며 그것들은 각각 현상학적 관점에서의 체험된 시간-"시간과 가족 간호(temporality and family caring) :하루하루를 대처해 나감(taking each day as it comes)" -과 체험된 관계 - "관계와 가족 간호(relationality and family caring) ; 타인의 창을 통해 세상을 바라봄(looking at the world through the other's window) "-에 비추어 간호 제공의 경험을 묘사하고 있다. "하루하루를 대처해 나감"은 간호 제공자들이 정신질환 가족의 발병 초기부터 돌보아야 하는 일에 연루됨으로부터 경험하는 현상학적 의미의 시간을 내포하고 있으며 그것은 어둠 속에서 허우적 거림(floundering in the dark), 하루 하루를 대처해 나감(taking each day as it comes), 경험에 의한 터득(learning by experience), 지속된 슬픔(on-going grieving) 그러고 휴식을 통한 자기 보존(self preserving through time out) 으로 특징 되어진다. 다른 한편, "타인의 창을 통해 세상을 바라봄"은 연구 참여자들이 그들만의 특별한 방법으로 이 세상을 살아가는 경험을 어떻게 인지하고 해석하는지를 보여 주었으며 그것은 바로 그들이 돌보는 사람의 눈을 통해서 였음을 나타내었다. 이 주제에서는 갈등의 아픔(being torn), 타인의 눈을 통합(through the eyes of the other), 두려움 속의 삶(living in fear of), 은폐 당함(being swept under the carpet), 도움을 얻기 위한 싸움(battling to get help) 그리고 차이를 느낌(feeling the difference)으로 설명되어졌다.
본 연구의 가장 주된 공헌은 전문 간호사들과 더불어 다른 모든 건강 전문 요원들에게 복잡하고 지속적인 노력을 요구하는 가족 간호의 본질에 대한 이해를 높일 수 있도록 가족 간호 자들의 체험과 욕구들을 그 나름대로 설명했다는 점이다. 본 연구는 또한 가족 간호 자들의 욕구로서는 정신질환에 대한 사회적 이해, 지지와 임시 간호제의 효과적 실시라는 것을 확인시켰으며, 이러한 것은 간호 수행면에서 뿐 만아니라 사회 정책면에서 그리고 앞으로의 연구 방향의 기틀을 제공했다고 할 수 있다.
This paper presents a field study that examined family members' experiences of caring for a relative with chronic mental illness, primarily based on the family caregivers' personal accounts of their caring practices. Since the deinstitutionalization movement of the late 1960s and the 1970s gained momentum there has been increasing emphasis on community based care for people with mental illness, which has resulted in a significant increase in family responsibilities in caring for disabled family members. In this regard, respite care services have played an important role in reducing caregiver burn-out and maintaining caregivers' physical and emotional health. Despite this, there has been a paucity of research on family carers of people with chronic mental illness and their perceptions of respite care. The purpose of this descriptive, interpretive study was to depict the lived world of caring within the individual family context and within the more extensive system of health care and community support services.
The design of the study was guided by the work of Van Manen(1990) and utilized hermeneutic phenomenological inquiry to explore taken-for-granted experiences of caring in the context of the persons' everyday lives, which helped the researcher acquire in-depth understanding of these experiences and describe the phenomenon of family caregiving in the context of chronic mental illness. To uncover the meaning of caregiving to these family members with regard to their ongoing committment to caring for their mentally ill relative, the study focused on the experience of caregiving from the perspective of fourteen family caregivers, in particular mothers, fathers and a daughter, and their views on respite care. Through two occasions of semi-structured, in-depth interviews, the participants were asked to describe what it is like to be a family caregiver of a person with chronic mental illness. Transcribed interviews were analysed by thematically describing and interpreting the meanings of the lived experience.
Two principal themes were extracted which describe the experience of caregiving in the light of phenomenological notions of lived time-temporality and funiiy caring: taking each day as it comes, and lived other -relationality and family caring : looking at the world through the other's window. Taking each day as it comes connoted lived time that the caregivers experienced from the beginning of their involvement in caring for their mentally ill relative, characterized by floundering in the dark, taking each day as it comes, learning by experience, ongoing grieving, and self-preserving through time out. On the other hand, looking at the world through the other's window showed how the study participants perceived and interpreted their experience of being in the world in a particular way, and that was through the eyes of the one they cared for. This was explicated through sub themes of being torn, through the eyes of the other, living in fear of, being swept under the carpet, battling to get help and feeling the difference. The main contribution of the present study is its explication of the family caregivers' experiences and needs in a way which can increase nurses' and other health professionals' appreciation of the complex, ongoing, and demanding nature of family caregiving. The study identified the caregivers' needs for understanding, support and respite, and these provided directions for nursing practice, social policy, and future research.
