The idea that serotonin was concerned in mental processes, and that deficiency or excess of this substance in the brain might be the cause of some mental diseases, was first proposed early in 1954 by Woolley and Shaw.
They have also observed that many antimetabolites of serotonin caused mental disturbances in man behavioral changes in animal, and recognized that some of their pharmacological action consisted in producing a deficiency of this hormone.
Although it was, at first, inferred from the psychotic effects of antiserotonins that mental disorder was the result of a deficiency of serotonin in the brain, a complication arose from the observation that many of these analogues of the substance also possessed some serotonin like activity.
Numerous reports have appeared in the literature citing evidence for quantitative and qualitative differences 03 urinary indole substances between normal subjects and schizophrenic patients, but no agreement has been established except the greater individual variations in schizophrenic patients.
In this study, it was attempted to measure the levels of urinary excretion of 5-HIAA, the principal metabolite of serotonin, in various types of schizophrenic patients with or without tranquilizer treatment, as well as evaluate the data with the normal urinary excretion levels. These might be summarized as follows;
1. There was no significant diflerences between schizophrenic patients and normal controls as far as the mean value of daily urinary excretion of 5-HIAA was concerned. However the schizophrenic patients, .specially the hebephrenia and deteriorated schizophrenics whose personality disintegration is the most severe one among the a11 schizophrenic subtypes, showed much greater person-to-person variabiljty than did the normal subjects.
2. The schizophrenic patients who were recieving chlorpromazine therapy with daily dosages ranging From 100 to 600 mg for a considerable period showed remakable decreases in the levels of urinary 5-HIAA excretion.
3. The schizophrenics recieving reserpine treatment with daily dosages more than 2.0 mg for more than two weeks showed more marked decreases in the concentration of urinary 5-BIAA and the total daily excretion of it.
