Ⅰ. 연구 배경 및 목적
□ 연구 배경 및 필요성
ㅇ 우리나라는 1990년대 중반 이후 급격한 경제발전과 산업화, 그리고 도시화로 인한 인위적 대기오염물질의 배출량이 증가함
- 대기질을 개선하고 대기오염으로 인한 건강부담을 경감시키고자 국가적인 차원에서 다양한 환경정책들을 시행
ㅇ 미세먼지(PM2.5)는 입자의 크기 2.5㎛ 이하로 매우 작아 폐포까지 깊숙이 침투하여 인체 위해성이 높은 것으로 보고
ㅇ 미세먼지(PM<sub>2.5</sub>)의 발생원과 구성성분에 따라 건강위해성에 차이가 있으므로, 인체 위해 측면의 관리를 위해서는 미세먼지(PM<sub>2.5</sub>)의 구성성분에 따른 건강영향에 대한 과학적 근거가 필요

Ⅱ. 미세먼지의 구성성분 및 건강영향 사례연구
□ 미세먼지(PM<sub>2.5</sub>)는 50개 이상의 화학적 성분의 복합 혼합물로, 발생원에 따라 미세먼지(PM2.5) 구성성분의 차이
ㅇ 탄소성분(EC, OC), 금속성분(Al, Ba, Be, Ca<sup>2+</sup>, Cd, Cr, Cu, Fe, Mg<sup>2+</sup> 등), 이온성분(NO<sub>3</sub><sup>-</sup>, SO<sub>4</sub><sup>2-</sup>, NH<sup>+</sup>, Cl<sup>-</sup>, Br<sup>-</sup>, Na<sup>+</sup>, K<sup>+</sup>, Ca<sup>2+</sup>, Mg<sup>2+</sup>) 등 다양한 물질들로 구성
ㅇ 국내 미세먼지(PM<sub>2.5</sub>)의 주요성분은 질산염, 황산염, 암모늄, 유기탄소, 원소탄소로 나타났으며 지역별로 각 주요성분들이 차지하는 비율은 상이
- 서울, 인천과 광주의 경우 유기탄소가 가장 큰 비중을 차지하였고, 대전, 부산, 청주 등의 도시에서는 황산염의 비중이 가장 큰 것으로 나타남
- 미세먼지(PM<sub>2.5</sub>) 발생 기여도 분석에 있어 상이한 방법론을 적용하였으며 오염원의 분포는 계절이나 기상현상에 따라 달라지는 것으로 나타남

ㅇ 미국 건강영향연구소(HEI: Health Effects Institute)는 미세먼지 구성성분에 의한 건강영향에 대한 통합적 역학 및 독성 연구 등 4개의 연구로 구성
- 시계열 분석 결과 미세먼지(PM<sub>2.5</sub>)와 가스상 오염 물질의 성분은 미세먼지(PM<sub>2.5</sub>) 질량 농도와 같이 건강영향에 밀접한 연관성을 보임

Ⅲ. 미세먼지 구성성분의 특성분석
□ 서울시 미세먼지(PM<sub>2.5</sub>) 농도 특성 파악
ㅇ 2015~2017년 서울시 미세먼지(PM<sub>2.5</sub>)의 연평균 농도 값은 2016년에 26.29㎍/㎥로 가장 높았음
ㅇ 서울시 북동지역의 미세먼지(PM<sub>2.5</sub>) 농도분포가 다른 지역보다 낮은 패턴을 보였으며 남서지역의 미세먼지(PM<sub>2.5</sub>) 농도가 다른 지역보다 높은 패턴을 보임
□ 2015년과 2016년의 배출원별 전국과 서울시의 미세먼지(PM<sub>2.5</sub>) 배출특성 파악
ㅇ 전국 미세먼지(PM<sub>2.5</sub>) 총배출량은 2015년 98,806톤에서 2016년 100,247톤으로 증가한 반면 서울시의 경우 2015년(2,580톤)보다 2016년(2,524톤) 미세먼지(PM<sub>2.5</sub>) 배출량이 다소 감소
□ 2015년부터 2017년까지 서울시 미세먼지(PM2.5) 구성성분(탄소성분(OC, EC), 수용성 이온성분(NO<sub>3</sub><sup>-</sup>, SO<sub>4</sub><sup>2-</sup>, NH<sub>4</sub><sup>+</sup>), 금속이온성분(Ca<sup>2+</sup>, K<sup>+</sup>, Na<sup>+</sup>, Mg<sup>2+</sup>)의 특성 파악
ㅇ 미세먼지(PM<sub>2.5</sub>)대비 비율을 살펴보면 탄소성분의 OC 비율이 평균 26%로 가장 높았으며 이온성분의 NO<sub>3</sub><sup>-</sup>(20.54%)와 SO<sub>4</sub><sup>2-</sup>(16.57%)의 비율이 다음으로 높게 나타남
ㅇ OC의 계절별 변화는 겨울(7.89㎍/㎥) > 봄(6.36㎍/㎥) > 가을(6.62㎍/㎥) > 여름(5.13㎍/㎥) 순으로 나타남
ㅇ 미세먼지(PM<sub>2.5</sub>)와 비교적 높은 상관성(0.5이상)을 보인 것은 이온성분으로 중NO<sub>3</sub><sup>-</sup>(0.57)과 Cl<sup>-</sup>(0.84)이었으며 금속 이온성분 중 K<sup>+</sup>(0.79), Mg<sup>2+</sup>(0.65), Na<sup>+</sup>(0.67)임
Ⅳ. 미세먼지 구성성분별 건강영향 평가
□ 2015년부터 2017년까지 서울시를 대상으로 미세먼지(PM<sub>2.5</sub>)와 미세먼지(PM<sub>2.5</sub>) 구성성분의 농도 증가로 인한 사망영향을 평가
ㅇ 미세먼지의 구성성분에 따른 전체원인 사망영향과 심혈관계 사망영향을 정량적으로 평가하기 위해 시계열 연구를 적용

Ⅴ. 수용체 중심의 미세먼지 관리정책 방안
□ 인체위해성에 기반한 미세먼지 관리정책 국외 사례 검토
ㅇ 미국 남부 캘리포니아 지역은 대기자원위원회(California Air Resources Board)를 설립하여 자동차 배기가스를 규제. BC 농도가 1960년 대 이후 90% 가까이 감소하면서 매년 약 5,000명의 조기 사망을 예방
ㅇ 유럽연합은 대기환경 관리 및 대기질 개선을 위해 2001년 국가별 배출량 상한 지침(National Emission Ceilings Directive)을 채택하고 미세먼지(PM<sub>2.5</sub>) 배출 저감조치를 취할 때 건강피해를 야기하는 블랙카본(Black Carbon) 검토를 명시
□ 인체위해성에 기반한 미세먼지 관리로 패러다임의 전환
ㅇ 미세먼지 관리가 국민의 건강피해를 감소시키고 쾌적한 환경을 조성하기 위한 것으로 목적을 달성하기 위해서는 우선적으로 인체위해성에 기반한 미세먼지 관리로 패러다임의 전환 필요

- 미세먼지(PM<sub>2.5</sub>) 구성성분 농도를 측정하기 위해서는 막대한 재원과 전문 인력이 필요하지만, 미세먼지 관리의 기본적인 요소인 구성성분 정보를 파악하기 위해서는 구성성분 측정이 반드시 필요
- 구성성분과 같이 세분화되고 과학적인 사용이 필요한 항목에 대해서는 별도의 정보제공이 이루어질 수 있어야 함
ㅇ 미세먼지와 구성성분의 건강영향 평가
- 미세먼지(PM<sub>2.5</sub>) 에 대한 양적인 관리뿐만 아니라 질적 관리를 위해 미세먼지(PM<sub>2.5</sub>) 배출원의 배출요인, 배출량, 구성성분별 농도 특성이 분석되어야 하며 이를 통한 인구집단의 미세먼지(PM<sub>2.5</sub>) 노출과 위해도 분석 필요
ㅇ 인체위해 저감을 위한 배출원 맞춤형 관리대책 추진
- 미세먼지(PM<sub>2.5</sub>)와 구성성분에 대한 기여도를 고려하여 관리대책의 추진 가능 범위를 확인. 배출원의 위치, 배출오염물질과 같은 다양한 제한요인을 고려하여 지역별 배출원 맞춤형 관리대책 추진
- 국내에 타당한 분석방법을 적용하여 다양한 미세먼지(PM<sub>2.5</sub>)의 배출원 기여도를 보다 정확하게 평가할 필요가 있음
Ⅰ. Study Background and Purpose
□ Study Background
ㅇ In Korea, the emission of air pollutants has increased since the mid-1990s due to rapid economic development, industrialization, and urbanization.
- In order to improve air quality and reduce the health burdens caused by air pollution, various environmental policies are implemented at the national level.
ㅇ PM<sub>2.5</sub> has particle sizes smaller than 2.5μm and was reported to cause high risks in human health as it can penetrate deeply into the alveolar.
ㅇ There is a difference in health risks according to the source and constituents of PM<sub>2.5</sub>, so for the management of the human health risk, scientific evidence for the effects on health according to the constituents of PM<sub>2.5</sub> is needed.
□ Study Purpose
ㅇ The characteristics of the constituents of PM<sub>2.5</sub> were investigated and the health effects of PM<sub>2.5</sub> constituents were quantitatively evaluated.
ㅇ The qualitative management policy for PM<sub>2.5</sub> was proposed to prepare a prioritized management policy for the PM<sub>2.5</sub> constituents with high risks on human health.
Ⅱ. Case Studies on PM<sub>2.5</sub> constituents and Health Effects of PM<sub>2.5</sub> constituents
□ PM<sub>2.5</sub> is a composite mixture of more than 50 chemical constituents, and the constituents of PM<sub>2.5</sub> vary according to the source.
ㅇ PM<sub>2.5</sub> is composed of various chemicals such as carbonaceous components(EC, OC), trace metal(Al, Ba, Be, Ca<sup>2+</sup>, Cd, Cr, Cu, Fe, Mg<sup>2+</sup> etc.), and ions components(NO<sub>3</sub><sup>-</sup>, SO<sub>4</sub><sup>2-</sup>, NH<sup>+</sup>, Cl<sup>-</sup>, Br<sup>-</sup>, Na<sup>+</sup>, K<sup>+</sup>, Ca<sup>2+</sup>, Mg<sup>2+</sup>), etc.
ㅇ Major constituents of PM<sub>2.5</sub> in South Korea were nitrate salt, sulfate salt, ammonium, organic carbon, and elemental carbon, and the ratio of each major constituent vary according to region.
- Organic carbon was the dominating constituent in Seoul, Incheon and Gwangju, and sulfate salt was the dominating constituents in Daejeon, Busan, and Cheongju.
- Different methodologies were applied for contribution analysis of PM<sub>2.5</sub> sources, and distribution of the PM<sub>2.5</sub> source varied according to seasons or weather conditions.
□ Case analysis of studies on health effects of PM<sub>2.5</sub> constituents in US and Europe
ㅇ The Health Effects Institute in the United States conducted four studies on health effects of PM<sub>2.5</sub> constituents, including studies on integrated epidemiology and toxicity.
- Time series analysis showed that the constituents of PM<sub>2.5</sub> and gaseous pollutants were closely related to health effects along with PM<sub>2.5</sub> mass concentration.
ㅇ The Integrated Science Assessment of the US National Air Quality Standards studied the relationship between PM<sub>2.5</sub> constituents and mortality, and confirmed a relationship between concentration and ratio of PM<sub>2.5</sub> constituents and mortality through a comparison with the mass of PM<sub>2.5</sub>.
ㅇ MED-PARTICLES in Europe, with support of the European Commission from 2010 to 2013, identified the characteristics of PM<sub>2.5</sub> constituents in 14 cities in the Mediterranean countries.
□ Study of short-term exposure to PM<sub>2.5</sub> and its constituents and health
ㅇ Health risk of PM<sub>2.5</sub> constituents is different based on the region, the cause of death or disease, and season, etc.
Ⅲ. Characteristics of PM<sub>2.5</sub> Constituents
□ Concentration characteristics of PM<sub>2.5</sub> in Seoul
ㅇ Between 2015-2017, the annual average PM<sub>2.5</sub> concentration in Seoul was the highest in 2016 with 26.29㎍/㎥.
ㅇ In Seoul, the PM<sub>2.5</sub> concentration showed that northeastern area had the lowest concentration and southwestern area had the highest concentration.
□ PM<sub>2.5</sub> emission characteristics in Seoul and South Korea according to emission source during 2015 and 2016
ㅇ Total amount of PM<sub>2.5</sub> emission in South Korea increased from 98,806 tons in 2015 to 100,247 tons in 2016, while the amount of PM<sub>2.5</sub> emission in Seoul slightly decreased in 2016(2,524 tons), compared to 2015(2,580 tons).
□ Characteristics of PM<sub>2.5</sub> constituents in Seoul from 2015 to 2017
ㅇ A review on the constituents ratio in the PM2.5 showed that the ratio of OC, a carbon constituents was the highest with 26% in average, followed by the ratio of NO<sub>3</sub><sup>-</sup>(20.54%) and SO<sub>4</sub><sup>2-</sup>(16.57%).
ㅇ Seasonal change of OC was higher in the order of winter(7.89㎍/㎥) > spring(6.36㎍/㎥) > fall(6.62㎍/㎥) > summer(5.13㎍/㎥).
ㅇ The constituents showing relatively high correlations (0.5 and higher) with PM<sub>2.5</sub> were NO<sub>3</sub><sup>-</sup> and Cl<sup>-</sup> among the ion constituents and K<sup>+</sup>, Mg<sup>2+</sup> and Na<sup>+</sup> among the metal ion constituents.
Ⅳ. Assessment the Health Effects of PM<sub>2.5</sub> Constituents
□ The mortality effects of PM<sub>2.5</sub> and its constituents in Seoul from 2015 to 2017 were assessed.
ㅇ Time series study was applied to quantitatively evaluate the total death and cardiovascular death according to the composition of PM<sub>2.5</sub>.
ㅇ EC and OC statistically significantly increased the total cause mortality risk in the total age group and the age group over 65 years.
- The total mortality risk of EC and OC is higher than the mortality risk of PM<sub>2.5</sub>.
ㅇ Cland SO<sub>4</sub><sup>2-</sup> statistically significantly increased the risk of total death risk and cardiovascular death in the age group of less th an 65years.

ㅇ The California Air Resources Board was established in southern California, USA, to regulate vehicle emissions. BC concentrations have
declined by nearly 90% since the 1960s, preventing approximately 5,000 premature deaths each year.
ㅇ The European Union implemented the National Emission Ceilings Directive in 2001 to improve air quality and manage air environment and enforced the review of the Black Carbon which causes health risks when taking measures to reduce PM<sub>2.5</sub> emissions.
□ Paradigm shift to PM<sub>2.5</sub> management based on human health risks
ㅇ The management of PM<sub>2.5</sub> is to reduce the health damage of the people and to create a healthy environment. To achieve these objectives, there should be a paradigm shift first to the management of PM<sub>2.5</sub> based on human health risks.
□ Implementation of PM<sub>2.5</sub> management plan based on human health risks
ㅇ Measurement of PM<sub>2.5</sub> constituents and information
- Although measuring the concentration of PM<sub>2.5</sub> constituents requires a great deal of resources and professional personnel, PM<sub>2.5</sub> constituent measurement is essential because the constituents information is a basic factor in the PM<sub>2.5</sub> management.
- For items such as constituents that are subdivided and need scientific analysis, there should be separate information provided.
ㅇ Health effect of PM<sub>2.5</sub> and its constituents
- For qualitative management as well as quantitative management of PM<sub>2.5</sub>, emission factors, emission amounts, and concentration characteristics of PM<sub>2.5</sub> emission sources should be analyzed, and we estimate the association between PM<sub>2.5</sub> and its constituents and health.
ㅇ Implementing customized management measures for emission sources to reduce human health risk
- First, the scope of the management measures needs to be confirmed by considering the contribution of emission source to PM<sub>2.5</sub> and constituents. Then customized management measures for regional emission sources need to be implemented, considering various limiting factors such as the location of emission sources and emission pollutants.
- It is necessary to evaluate the contribution of various PM<sub>2.5</sub> emission sources more accurately by applying a more adequate analysis method for South Korea.
