Background:?Diabetic ketoacidosis (DKA) is usual acute metabolic complication in type 1 diabetes. Hyperglycemic hyperosmolar state/syndrome (HSS) is common in type 2 diabetes by acute decompensation. DKA is rare but can be occurred in type 2 diabetes. Insulin deficiency caused by acute stress such as severe illness, infection and infarction of tissue may induce DKA in type 2 diabetes. In emergency room (ER), every instances needs to differentiate the type of diabetes for proper treatment. The purpose of this study was to investigate the type-specific features and treatment of DKA patients visiting ER.?Methods:?We studied 144 patients who diagnosed acidosis by ER between January 2014 and December 2014. 34 patients were diagnosed with DKA. The definition of DKA is a disorder in diabetic patients that shows high ketone level and high anion gap metabolic acidosis. By reviewing the chart, DKA patients were divided into 2 groups depending on diabetic types; type 1 (n=20), Type 2 (n=11). Demographics, disease related characteristics, Laboratory data, treatment and hospital course was compared between diabetic types by T-test and χ2?using the SPSS statistics version 20.?Results:?DKA patients with type 2 diabetes were older and heavier than type 1 (p=0.01). And hypertension is often in type 2 (p=0.01). Most patients with type 1 diabetes were taken insulin therapy. And Type 2 diabetic patients were taken previously oral hypoglycemic agents (9.09%), insulin (36.36%), and combination therapy (27.27%). Ketone level was significant higher in type 1 diabetes (9363.86±748.93 umol/L, 1433.00±1725.26 umol/L, p=0.01). There was no difference in insulin administration dose to normalization of pH (p=0.877) and hospital course between diabetic groups.?Conclusions:?DKA in type 1 diabetes showed higher ketone level than type 2. DKA patients in type 2 were older and heavier than type 1 and had higher prevalence of hypertension. However there was no difference in treatment and hospital course between diabetic types. Favorable prognosis only pertains to proper insulin therapy, not to type-specific characteristics and treatment.
