Renoprotective effect of Sinkihwan-gamibang on puromycin aminonucleoside-induced nephrotic syndrome in rats

Nephrotic syndrome is one of the commonest kidney diseases and is characterized by hypertriglyceridemia, massive proteinuria, hypo-albuminemia and peripheral edema (1). Many of the complications of nephrotic syndrome, including atherosclerosis, thromboembolism, hypovolemia, and acute renal failure. Sinkihwan-gamibang was recorded in a traditional Chinese medical book named “Bangyakhappyeon (方藥合編)” and three prescriptions Sinkihwan, Geumgwe-sinkihwan, and Jesaeng-sinkihwan belong to Gamibang. Sinkihwan has been reported that it is used for complications of diabetes and treatment of glomerulonephritis with patients (2-3). However, the effects of Sinkihwan-gamibang on renal dysfunction has not been reported yet. The present study aims to investigate the role of Sinkihwan-gamibang on renal dysfunction in PAN-induced nephrotic syndrome model. Experimental nephrotic syndrome induced by injection of puromycin aminonucleoside (PAN) into rats is a well-established model of nephrotic syndrome. Experimental nephrotic syndrome was induced in male Sprague Dawley rat (SD rat) by femoral vein injection of PAN (50 mg/kg). The animals were divided into six groups of eight rats in each. As a result, Shingihwan-gamibang not only reduced the size of the kidneys increased by puromycin-induced nephrotic syndrome, but also decreased proteinuria and ascites, one of the symptoms of nephrotic syndrome. In addition, Shingihwan-gamibang was significantly ameliorated creatinine clearance (Ccr), creatinine (CRE) and blood urea nitrogen (BUN) in the plasma obtained. Shingihwan-gamibang reduced the Periodic acid schiff (PAS) staining intensity and glomeruli dilation and tubules fibrosis in glomeruli of nephrotic syndrome model. Shingihwan-gamibang inhibited protein and mRNA expression of NLRP3 inflammasome including NLRP3, ASC, and pro-caspase-1 in nephrotic syndrome rats. These findings indicate that treatment of Sinkihwan-gamibang have protective effects against renal dysfunction in PAN-induced nephrotic syndrome rats. Sinkihwan-gamibang may be potential therapies.
