두드러기의 임상 형태에 따른 혈중 호산구 및 IgE치 = Peripheral Blood Eosinophil and Serum IgE in Different Types of Urticaria
Background : Common environmental allergens play a role in the pathogenesis of urticaria via type I hypersensitivity reaction. Changes in the number of easinophils and serum IgE in circulating blood in various allergic disorders have been reparted by several investigators. However, there are only a few reports about the measurement of those numbers according to a clinical type of urticaria.
Objective : This study was undertaken to measure the peripheral blood eosinophil and serum IgE levels in 514 patients with different types of urticaria.
Method : We measured the peripheral blood eosinophil and serum IgE levels in 514 patients who visited the Department of Dermatology, Kyungpaok National University Hospital from November 1993 to December 1996. Of these patients there were 122 with acute urticaria (23.7%), 160 with chronic urticaria (31.2%), 164 v,~th dermagtaphism (31.9%), 11 with acute urticaria plus dermographism (2.1%) and 57 with chronic urticarla plus dermograpliism (11.1%).
Results : The results were as follows.
1. Ages of the patients ranged from 1 to 85 years old. The mean age of the patients with acute urticaria was 30.7 years and this mean age was lower than that (34.7 years) of all patients.
2. Mean serum IgE level in all patients was 248±299IU/ml, higher than the reference value (200IU/ml%), by enzyme immunoassay. Among the five groups, serum IgE levels in the groups with acute urticaria were higher than those of other groups.
3. Median level for eosinophils was 0.19±0.35(×109/L) in absolute count and 2.6±3.9% in percentage. It was within normal limits.
4. Rates of patients with peripheral blood eosinaphil and serum IgE levels exceeding upper normal limits were 7.6% in absolute eosinophil count, 6.5% in percentage of eosinophils and 32.5% in serum IgE, respectively.
5. Statistically significant correlations were not observed between eosinophil and serum IgE levels, fox the most part.
6. The number of urticaria patients combined with other allergic diseases was 50 (9.7%). Conclusion : In patients of urticaria, the rate (32.5%) of patients with levels exceeding upper normal limits in serum IgE was higher than that in the peripheral eosinophl count. Especially in cases of acute urticaria, serum IgE was raised.
