최근 5년간(1998~2002년) 동국대학교 경주병원에서 분리된 각종 병원성 세균의 항균제 내성양상 = The trend of antimicrobial resistance to bacteria isolated from Dongguk University Kyeongju Hospital between 1988 and 2002
목적 : 감염성 질환의 원인균이나 항균제 감수성 양상은 시대, 지역, 병원의 특성 그리고 대상 환자에 따라 달라질 수 있으므로, 저자들은 1998년부터 2002년까지 최근 5년간 동국대학교 경주병원에서 분리된 주요 병원성 세균의 양상과 항균제 내성율 변화를 분석하여 감염성 질환 환자에게 경험적 항균제 치료시 적절한 항균제를 선택하고, 또 향후 항균제 사용관리의 기본적인 자료로 이용하고자 본 연구를 시작하였다.
대상 및 방법 : 1998년 1월부터 2002년 12월까지 동국대학교 경주병원을 내원한 환자들 중에서 세균배양 검사가 의뢰되어 진단검사의학과에서 병원성 세균으로 분리 동정되었던 주요 그람음성 간균 및 그람양성 구균에 대하여 항균제 감수성 검사를 시행하였다. 세균배양검사가 의뢰된 검체에서 균의 분리 및 동정은 통상적인 방법을 따랐으며, 분리된 균주에 대한 항균제 감수성 검사는 Kirby-Bauer 변법에 의한 표준 디스크확산법으로 시행하였다.
결과 : 1998년부터 2002년까지 5년간 동국대학교 경주병원에서 분리된 주요 세균별 항균제 내성율은 다음과 같았다. E. coli는 cephalothin 33~51%, ceftazidime과 aztreonam 및 amilkacin에 2~10%, gentamicin 27~34%, ciprofloxacin 18~34%로 전국 다른 병원과 유사하였다. K. pneumoniae는 cephalothin 29~55%, ceftazidime과 aztreonam에 14~46%, amikacin 18~27%, gentamicin 5~38%, ciprofloxacin 1~13%의 내성율을 보였다. E cloacae는 gentamcin에 15~29%, ciprofloxci에 2~13%로 내성율이 낮았다. 그러나 E. cloacae는 1998년에서 2000년 사이에 ceftazidime 과 aztreonam에 61~84%, amikacin에 21~77%로 아주 높았지만 2001년 이후 39~42%와 9~10%로 전국 평균과 비슷하게 내성율이 급격히 낮아졌다. S. marcescens는 amikacin에 49-60%, aztreonam에 35~60%, ciprofloxacin에 34~46%의 내성률을 보여 전국 다른 병원의 평균보다 높았으며 특히 Ceftazidime에 대한 내성율은 98년과 99년에 56%와 51%로 아주 높았지만 2000년 이후 7~l4%로 크게 감소하였다. P. aeruginosa의 항균제 내성율은 gentamicin 37~59%, ciprofloxacin 30~42%, imipenem과 amikacin 및 aztreonam에 대해서는 10~40%, 9~30%, 4~l7%, ceftazidime에 10~14%였으며 특히 imipenem과 amikacin에 대한 내성율은 2001년 이후 10-20%와 9-15%로 오히려 감소되었다. 또한 본원에서는 imipenem내성 장내 세균은 분리되지 않았으며 2001년과 2002년에 분리된 ESBL생성 E. coli와 K. pneumoniae 그리고 E. cloacae의 분리율도 1.8~2.2%, 6.8~8.6% 그리고 5.4~11.4%로 전국 12개 병원간의 평균 분리율에 비해 현저히 낮았다. 주요 그람양성 구균의 균주별 항균제 내성율은 S. aureus가 ciprofloxacin에 52~66%로, oxacillin에 56~69%로 연도별 내성율이 점차 증가되어 2001년 이후 MRSA의 분리율이 전국 다른 대학병원의 평균분리율과 비슷해졌으며, vancomycin이나 teicoplanin에 내성인 S. aureus나 CNS균주는 분리되지 않았다. E. faecalis은 1998년에서 2000년 사이에 vancomycin과 teicoplanin에 대한 내성율이 38~45%로 아주 높았지만 2001년 이후에는 vancomycin에 14~16% teicoplanin에 9~18%로 전국 평균과 유사한 내성율을 보였다. S. pneumoniae는 2001년 이전에 51 ~60%이던 oxacillin 내성율이 2002년에 70%로 높아졌다.
결론 : 최근 5년간 동국대학교 경주병원에서 분리된 주요 병원성 세균이 항균제 내성율이 1998년~2000년에 비해 2001년과 2002년에 전국 평균과 유사한 내성율로 일정하게 유지되거나 일부 균주에서는 항균제 내성율이 감소하는 양상을 보였다.
Background: Since the causal bacteria of infectious diseases or the antimicrobial sensitivity aspects can be varied by the times, regions, hospital characteristics, subjective patients, and the periodical analysis. Therefore, we analyzed the changes of the main pathogenic bacterial aspects, isolated in Dongguk Univ. Kyeongju Hospital for the latest 5 years from 1998 to 2002, and the antimicrobial resistance rates, to select the proper antimicrobial agents for the infectious disease patients during the empirical treatment. Moreover, this study was initiated to usuage the results as the fundamental data for the pertinent use and control of the antimicrobial agents in the future.
Methods: We performed the antimicrobial sensitivity tests for the bacteria isolated from the department of Laboratory medicine through the bacterial culture tests requested for the patients who were admitted to Dongguk Utiv. Kyeongju Hospital from January 1998 to December 2002. The isolation and identification of the bacteria were followed by the general methods, and the antimicrobial sensitivity test was conducted according to the standard disk diffusion test by the modified Kirby-Bauer method.
Results: The antimicrobial resistance rates of the pathogenic organisms isolated from Dongguk Univ. Kyeongju Hospital for 5 years from 1998 to 2002 were as follows. E. coli showed the resistance rate 33-5l% to cephalothin, 2-10% to ceftazidime and aztreonam, and amikacin, 27-34% to gentamicin, and 18-34% to ciprofloxacin, which were similar to the rates of other hospitals in Korea. K. pneumoniae exhibited the rate 29-55% to cephalothin, 14-46% to ceftazidime and aztreonam, 18-27% to amikacin, 5-38% to gentamicin, and 1-13% to ciprofloxacin, and especially, the resistance rate to aztreonam was highly shown to be 39% in 1999 and 37% in 2000, but was remarkably decreased to 23% since 2001. The resistance rates of E. cloacae were relatively low as 15-29% to gentamcin and 2-13% to ciprofloxacin, however, the rates of antimicrobial resistance to ceftazidime and aztreonam and to amikacin were respectively quite high as 61-84% and 21-77% from 1998 to 2000 but were rapidly decreased to be 39-42% and 9-l0% each as similar to the national averages since 2001. S. marcescen showed the higher resistance rates as 49-60% to amikacin 35-60% to aztreonam, and 34-46% to ciprofloxacin in our hospital than other hospitals, and particularly, its resistance rate to ceftazidime was very high as 56% in 1998 and 51% in 1999 but was considerably decreased to 7-14% since 2000. The anitimicrobial resistance rates of P. aeruginosa were 37-59% to gentamicin, 30-42% to ciprofloxacin, 10-40% to imipenem, 9-30% to amikacin, 4-l7% to aztreonam, and 10-14% to ceftazidime, and expressly, the resistance rates to imipenem and amikacin were rather decreased each to 10-20% and 9-15% since 2001. Moreover, our hospital did not isolate an imipenem resistant enterobacteriaceae and the ESBL-producing E. coli, K. pneumoniae and E. cloacae isolated in 2001 and 2002 were notably lower as 1.8-2.2%, 6.8-8.6%, and 5.4-11.4% for each isolate. S. aureus gradually showed the annually increased resistance rates as 52-66% to ciprofloxacin and as 56-69% to oxacillin, and the isolation rate of MRSA since 2001 was almost equal to other university hospitals of this country. S. aureus or CNS strain tolerant to vancomycin or teicoplanin was not isolated. However, E. faecalis had significantly high resietanre to vancomycin and teicoplanin as 38-45% between 1998 and 2000 but showed the demrased resistance rates as 14-16% to vancomycin and as 9-18% to teicoplanin since 2001, which were similar to the national averages. S. pneumoniae showed the incrased rate of antimicrobial resistance to oxacillin from 51-60% in before 2001 to 70% in 2002.
Conclusions: The rates of antimicrobial resistance of the pathogenic bacteria isolated from Dongguk Univ. Kyeongju Hospital for the latest 5 years were regularly maintained to be similar to the national averages in 2001 and 2002, compared with the rates in 1998-2000, or even some bacterial strains showed rather decreased rates of antimicrobial resistance.
