본 연구는 관상동맥 질환을 가진 남성과 여성의 삶의 질 차이를 확인하고 사회적 지지, 우울 및 인구 사회학적 특성을 중심으로 그 영향 요인에서 차이를 파악하고자 시행한 서술적 조사연구이다. 대상자는 관상동맥 질환으로 관상동맥 중재술이나 관상동맥 우회술 후 1개월이 지난 자 및 심장 발작(heart attack)을 경험하고 약물 치료 중인자로 의사소통이 가능하며, 본 연구에 대해 동의한 남성 67명, 여성 65명이다. 자료수집 기간은 2008년 8월 4일부터 9월 20일까지로 전화를 통해 연구에 대해 설명한 후 외래 방문시 동의서를 작성 후 설문지를 완성토록 하였다.
삶의 질은 Ware, Kosinki 와 Dewey(2001)의 도구를 고상백(2003)이 한역한 Short Form 36-items(SF-36)도구로 측정하였다. 사회적 지지는 Brandt와 Winert(1981)가 개발한 것으로 탁영란, 이희영(1997)이 번안하여 사용한 Personal Resource Questionnaire-part(II)도구를 사용하였으며, 우울은 The Center for Epidemiologic Studies Depression Scale 한국판으로 측정하였다.
자료분석은 SPSS 통계 프로그램을 이용하였으며, 대상자의 일반적 특성, 삶의 질, 사회적 지지, 우울은 실수, 백분율, 평균과 표준편차로 분석하였고, 사회적 지지와 우울 및 삶의 질 상관관계는 Pearson`s Correlation coefficient로 분석하였으며, 삶의 질 영향 요인 분석은 다중 회귀분석(multiple regression)으로 분석하였다.
연구결과를 요약하면 다음과 같다:
1) 성별에 따른 삶의 질, 사회적 지지, 우울 비교 결과 삶의 질 평균 점수는 남성은 67.22, 여성은 54.49이며, 사회적 지지 점수는 남성의 평균은 4.82 였고 여성의 평균은 4.83(t=-0.35, p=0.972)으로 나타났다. 우울점수는 남 성이 평균 9.26, 여성은 13.4(t=-2.72, p=0.007)였다. 삶의 질은 남성이 여성 보다 유의하게 높았고, 우울은 여성이 남성보다 유의하게 높은 것으로 나 타났으나 사회적 지지에서는 남성과 여성에서 차이가 없는 것으로 나타났 다.
2) 삶의 질과 사회적 지지, 우울의 상관관계를 분석한 결과 남성의 삶의 질과 사회적 지지는 양의 상관관계를 나타냈고 통계적으로 유의하였고(r=0.474, p=0.000), 삶의 질과 우울은 음의 상관관계를 보였고 통계적으로 유의하였 다(r=-0.675, p=0.000). 또한 사회적 지지와 우울은 음의 상관관계로 통계적 으로 유의하게 나왔다(r=-0.547, p=0.000). 여성의 경우에도 삶의 질과 사회 적 지지는 양의 상관관계로 통계적으로 유의하게 나왔으며(r=0.629, p=0.000), 삶의 질과 우울은 음의 상관관계로 통계적으로 유의하게 나왔고 (r=-0.673, p=0.000), 사회적 지지와 우울 사이에는 유의한 음의 상관관계가 있었다(r=-0.672, p=0.000).
3) 일반적 특성에 따른 삶의 질 비교 결과 대상자들의 일반적 특성 중에서 삶 의 질에서 유의한 차이를 보인 변수는 성별, 교육수준, 직업유무, 월수입, 관상동맥 질환 외 기타질환 유무, PTCA 시술 횟수 및 가족력으로 나타났 다. 이들 변수 중 대상자의 삶의 질에 영향을 주는 요인은 우울, 성별, 사 회적 지지였고 이들 변수들은 삶의 질 변량의 57.9%를 설명하는 것으로 나 타났다.
4) 남성의 일반적 특성에 따른 삶의 질 비교 결과 유의한 차이를 보인 변수는 직업 유무였으나 삶의 질에 유의한 영향을 미치는 변수로는 우울만이 유의 하게 나왔고 삶의 질 변량의 51.9%를 설명하였다.
5) 여성의 일반적 특성에 따른 삶의 질 비교 결과 유의한 차이를 보인 변수는 관상동맥 질환 외 기타질환 유무, PTCA 시술 횟수였으며, 삶의 질에 유의한 영향 요인은 사회적 지지와 우울이었으며 이 두 변수는 전체 삶의 질 변량 의 50.9%를 설명하였다.
이상과 같이 여성은 남성보다 삶의 질이 낮으며, 우울정도가 높은 것으로 나타났고 삶의 질 영향 요인에 있어서도 남성과 여성에 차이가 있었다. 따라서 관상동맥질환자의 삶의 질 향상을 위한 간호중재 개발에서 남성과 여성에서 다른 전략과 접근이 필요하다.
이상의 연구결과를 토대로 다음과 같이 제언한다:
1) 성별에 따라 삶의 질에 영향을 주는 요인에 차이가 있음을 알고 그에 따 른 차별화된 간호중재 프로그램 개발과 프로그램의 효과 검증을 위한 후속 연구를 제언한다.
2) 여성 대상자들의 사회적 지지 증진을 위한 자조모임 구축마련과 효과에 대 한 연구가 이루어져야함을 제언한다.
3) 남성 대상자들의 우울에 대한 인식을 높이고 퇴원시 간호교육이 이루어져야 함을 제언한다.
4) 본 연구 결과의 일반화를 위해 성별에 따라 삶의 질에 영향을 주는 요인에 대한 대상자 숫자를 증가시킨 반복 연구를 제언한다.
This is a descriptive study, which aims at addressing the differences between the quality of life, social support, depression and demographical features of males and females with coronary artery diseases, and at identifying differences in influencing factors of quality of life in males and females coronary artery disease. The subjects of the study were 67 males and 65 females with coronary artery diseases. Subjects who satisfied following criteria were recruited: 1) subjects who had undergone one of three treatment procedures such as percutaneous transluminal coronary angioplasty (PTCA), coronary artery bypass graft (CABG) surgery or medication therapy after heart attack, 2) subjects who had progressed at least one month after treatment initiation, 3) subjects who were able to communicate and who agreed to participate in the study. Prior telephone contact was made, the purpose of this study was informed and asked participation in the study. When they visited outpatient clinic for regular check-up, they were asked to fill out the questionnaire and informed consent.
To measure the quality of life, SF-36 tool was used, which was made by Ware, Kosinki & Dewey (2001) and was translated to Korean version by Goh (2003). The social support was measured by PRQ-part II tool, which was developed by Brandt and Winert (1981) and adapted by Tak and Lee (1997), and the depression was measured by the Korean version of CES-D Scale.
The data were analyzed by SPSS statistics program. The subjects’ general characteristics, quality of life, social support, and depression were analyzed using frequency, percentage, mean and standard deviation. The relationships among their social support, depression and quality of life were analyzed by Pearson’s Correlation coefficients, and influencing factors of quality of life were analyzed by multiple regression.
The study results can be summarized as the following:
1) The mean score of quality of life was 67.22 for males and 54.49 for females. The mean of social support for males was 4.82 and that of females was 4.83 (t=-0.35, p=0.972). The mean of depression score was 9.26 in case of males, and 13.4 in case of females (t=-2.72, p=0.007). The males’ quality of life was significantly higher than those of females. The score of depression for females was significantly higher than those of males, but there was no significant difference in terms of social support between males and females.
2) In terms of the correlations among the quality of life, social support, and depression, the quality of life and social support of males showed a positive correlation and was statistically significant (r=0.474, p=0.000). Their quality of life and depression displayed a statistically significant negative correlation (r=-0.675, p=0.000). Also the correlation between social support and depression was statistically significant negative correlation (r=-0.547, p=0.000). In case of females, the correlation between the quality of life and social support was positive and statistically significant (r=0.629, p=0.000), and the correlation coefficient between the quality of life and depression was negative and statistically significant (r=-0.673, p=0.000). The correlation coefficient between social support and depression was also negative and statistically significant (r=-0.672, p=0.000).
3) When the quality of life was compared according to the overall subjects’ general characteristics, the variables that showed significant difference in quality of life were gender, educational background, employment status, monthly income, the existence of other diagnosis, the number of undergoing PTCA and family history. Among them, depression, gender, and social support were significant influencing factors to the subjects’ quality of life, and these variables explained 57.9% of the variance of their quality of life.
4) The results of comparing the quality of life according to the general characteristics of males showed that there was significant difference in quality of life between who had job and who had not. In multiple regression analysis, depression was significant predictor of quality of life for male and it explained 51.9% of the variance of their quality of life.
5) When the females' quality of life was compared according to their general characteristics, there was significant difference in quality of life between women who had other health problems or diseases and women who had not any other health problems and the number of undergoing PTCA. In multiple regression analysis, social support and depression were significant influencing factors and these two variables explained 50.9% of the total variance of their quality of life.
As stated above, female patients were revealed to have lower quality of life and higher degree of depression than male patients. As for the elements affecting the quality of life, in case of males, depression explained 51.9% of the total variate of their quality of life, and in case of females, social support and depression explained 50.9% of the total variate of their quality of life.
Based on the study results, the following suggestions can be presented:
1) The fact needs to be recognized that the variables which affect the quality of life are different between male patients and female patients, and therefore a gender-differentiated nursing intervention program should be developed, and a study should be conducted to verify the effectiveness of the program.
2) Self-help groups need to be formed to improve the social support for the female patients and studies should be done on their effectiveness.
3) Deeper understanding on the male patients’ depression is needed to care for male patients and nursing intervention or counselling program should be developed for them.
4) Research should be conducted repeatedly on the factors that affect the patients’ quality of life, depending on their gender with larger sample size.
