Research Objective:
In October 2011, the Korean Government applied a new differential cost-sharing policy to the outpatients who get drug prescriptions at general hospitals or tertiary hospitals with non-critical 52 types of diseases. This policy emphasized increasing the coinsurance rate on pharmaceutical services to 50% in the tertiary care hospital and 40% in the general hospital. The purpose of this study is to evaluate the impact of a new cost-sharing policy on hypertension patients through 8 years (2011.10∼2019.12), tracing patients' medical use aspects, and present a policy proposal for the health insurance scheme.
Research Method:
The research subjects were 505,052 adults (≧21 years of age) with national health insurance insured who have visited tertiary or general hospitals with hypertension (I10.9, I10.0) in the pre-policy year.
The author defined shifting patterns of patients when they use medical services after policy implemented as 4 Types: patients who remain at tertiary or general hospitals (Type Ⅰ); patients who moved from tertiary or general hospitals to hospital or clinics (Type Ⅱ); patients who moved back from hospitals or clinics to tertiary or general hospitals again (Type Ⅲ); patients who moved back and forth between hospitals or clinics and tertiary or general hospitals (Type Ⅳ).
Independent variables consist of sociodemographic and health status variables in the one pre-policy year. Sociodemographic items are gender, age, beneficiaries' eligibility, health premium quintile, and geographic region. In the pre-policy year, health status items are whether patients have handicaps, special estimate cases, or complications of hypertension, and Charlson Comorbidity Index (CCI) they have. Independent variables are MPR (Medication Possession Ratio) of patients and the rate of complication of hypertension between Oct. 1, 2001, and Dec. 31, 2019.
The author performed χ2 tests to examine differences in sociodemographic and health status variables by shifting types of patients. Furthermore, the author performed multinomial logistic regression to analyze the impact of sociodemographic and health status items in the pre-policy year on patients' shifting types. To identify the impact of shifted types of patients on the MPR (Medication Possession Ratio) and the rate of complication of hypertension, the author performed logistic regression.
Research Results:
Of the 505,052 patients, 156,044 (30.9%) patients remained at tertiary or general hospitals (Type Ⅰ), 246,960 (48.9%) patients shifted from tertiary or general hospitals to hospitals or clinics (Type Ⅱ), 34,678 (6.9%) patients moved back from hospitals or clinics to tertiary or general hospitals again (Type Ⅲ), and 67,370 (13.3%) patients moved back and forth from the hospitals (or clinics) to tertiary (or general) hospitals (Type Ⅳ). Thus, the primary outcome is that this policy appears to have a positive effect on inducing patients in tertiary (or general) hospital shift to hospitals or clinics.
As compared to patients who shifted to hospitals or clinics (Reference Group), the patients who are remaining at the tertiary or general hospitals have a higher tendency to use tertiary or general hospital services when they are male (OR:1.4), under the age of 65 (OR:1.03∼1.16) than over 75, higher-income group (OR:1.08∼1.18), handicapped (OR:1.1), patients with higher Charlson Comorbidity Index (OR:1.02∼1.15) and complications of hypertension (OR:1.1).
The key finding of the current study is that there were differences between patient groups in the Medication Possession Ratio (OR:1.2) and the rate of complication of hypertension. As compared to patients who remained at tertiary or general hospitals, patients who shifted from tertiary or general hospitals to hospitals or clinics have higher Medication Possession Ratio (OR:1.2) and a lower rate of complication of hypertension (OR:0.96).
Policy Recommendation:
Some important policy recommendations based on results of analysis are as follows:
Firstly, it is necessary to strengthen education and publicity for patients. Despite implementing the differential cost-sharing policy, about 30% of hypertensive patients still use medical institutions at the level of general hospitals or higher. Patients with low severity or without complications need to actively participate in chronic disease management programs in primary medical institutions.
Secondly, the current study found a good result of the Medication Possession Ratio (74.8%). Nevertheless, we have to continuously give patients incentives in chronic disease management programs to enhance drug compliance through their voluntary attitude changes.
Thirdly, there is an important need regarding education for medical personnel who manage chronic disease patients in primary institutions. In order to manage complications, the efforts of patients are also important, but it is necessary to prevent complications through more thorough education for medical personnel in charge of chronic disease management.
Fourthly, even for patients with mild chronic diseases such as high blood pressure, it is necessary to take exceptional measures when treatment at a large hospital is deemed necessary because they have other serious diseases.

정부는 대형병원으로의 환자 쏠림현상을 완화하고자 2011년 10월 종합병원급 이상 의료기관을 대상으로 52개 외래 경증질환에 대해 약제비 본인부담차등정책을 시행하였다. 본 연구는 52개 질환중 고혈압 환자에 대해서 정책 시행이후 8년간(2011.10∼2019.12)까지의 이동패턴 등에 대한 추적분석을 통하여 정책의 영향을 파악하고 제도 발전을 위한 정책방향을 제언하고자 수행하였다.
연구대상자는 2010년 10월 1일부터 2011년 9월 30일까지 고혈압(I10.9, I10.0)을 주상병으로 종합병원 및 상급 종합병원에서 진료를 받은 21세 이상 건강보험 대상자(505,052명)이다. 정책 시행이후 연구대상자의 요양기관 종별 이동 현황을 다음과 같이 4가지 유형 즉, 종합병원급 이상 유지군, 병·의원 이동군, 종합병원급 이상 종별 회귀군, 종합병원급 이상과 병·의원 반복 이용군 등으로 구분하였다.
독립변수는 인구사회학적 요인과 지난 1년간 건강 상태 요인으로 구분하였다. 인구사회학적 요인으로는 성별, 연령, 건강보험 직역구분, 보험료 분위, 지역으로 구분하였다. 지난 1년간 건강상태요인으로는 장애여부, 동반상병지수(Charlson Comorbidity Index, CCI), 산정특례 질환 여부, 고혈압 관련 합병증 여부 변수를 이용하였다. 종속변수는 2011년 10월 1일부터 2019년 12월 31일까지 연구대상자를 추적 관찰하여 투약 순응률, 합병증 발생률로 정의하였다.
인구사회학적 요인 및 과거 건강상태요인과 요양기관 종별 환자 이동 패턴을 알아보기 위하여 기술통계 분석과 χ2-검정을 실시하였다. 대상자의 특성 요인들이 의료기관 종별 이동 패턴에 미치는 영향을 파악하기 위하여 다항로지스틱 회귀분석을, 요양기관 종별 환자 이동 패턴이 투약 순응률 및 합병증 발생에 미치는 영향을 알아보기 위하여 로지스틱 회귀분석을 하였다.
전체 연구대상자 505,052명의 의료기관 종별 이동 패턴 분석 결과, 종합병원급 이상 유지군은 156,044명(30.9%)이었고, 병·의원급 이동군은 246,960명(48.9%), 종합병원급 이상 종별 회귀군은 34,678명(6.9%), 종합병원급과 병·의원급 반복 이용군은 67,370명(13.3%)으로 나타나 정책의 영향이 큰 것으로 나타났다.
제도 시행 이전의 대상자 특성이 의료기관 종별 이동 패턴에 영향을 미치는 요인 분석을 실시한 결과, 여성에 비해 남성이(1.4배), 75세 이상 고령에 비해 65세 미만의 연령(1.03∼1.16배), 고소득층(1.08∼1.18배), 지난 1년간 장애를 가지고 있는 대상자(1.1배), Charlson 동반상병지수 지수가 높은 자(약 1.02∼1.15배), 고혈압 관련 합병증을 가지고 있는 자(1.1배) 일수록 병·의원보다는 종합병원급이상 의료기관을 지속적으로 이용하는 경향을 보였다. 또한 제도 시행이후 종합병원급 이상 유지군에 비해 병·의원급으로 이동한 대상자들의 투약 순응이 1.2배 높은 경향을 보였으며, 합병증 발생률은 0.96배로 낮은 경향을 보였다.
분석결과에 기초한 정책제안은 다음과 같다.
첫째, 환자들에 대한 교육과 홍보의 강화가 필요하다. 제도시행에도 불구하고 약 30%에 달하는 고혈압 환자가 여전히 종합병원급 이상 의료기관을 이용하고 있어 중증도가 낮거나 합병증 없는 환자는 일차의료기관 중심의 만성질환관리사업에 적극적으로 참여시킬 필요가 있다.
둘째, 고혈압 치료의 순응도를 높이는 것이 중요하다. 본 연구에서 투약 순응률은 74.8%를 나타내었다. 일차의료기관에서의 투약순응률을 높이기 위해서는 만성질환관리사업에 참여하는 환자에게 인센티브를 지속 제공하여 행위변화를 유도하는 것이 필요하다.
셋째, 일차의료기관에서의 만성질환 환자를 관리하는 의료인에 대한 교육이 필요하다. 합병증을 관리하기 위해서는 환자의 노력도 중요하지만 만성질환관리를 담당하는 의료인들에 대한 보다 철저한 교육을 통해 합병증을 예방하는 것이 필요하다.
넷째, 고혈압 등 경증질환을 가진 환자라도 타 중증질환이 있어 대형병원에서의 진료가 필요한 경우 예외적 조치를 취할 필요가 있다.
