As the elderly population increases, there is growing interest in studies reporting that dietary life is closely related to the development of chronic diseases, such as cognitive impairment and cardiovascular disease. In previous studies, mild cognitive impairment was defined as an intermediate stage between normal cognitive impairment and progression of Alzheimer's dementia as age increased and continued to require the establishment of practical guidelines to manage risk factors that accelerated disease progression. Recently, many studies have been conducted to analyze the effects of foods or dietary patterns, such as Mediterranean diet and MIND diet, on memory and cognitive function. However, there is a lack of consistency in outcomes for establishing guidelines for disease prevention and management. Therefore, it is necessary to quantify the significant variation of food intake in the large populations.
Study 1 was conducted on 276 participants aged 50 or older who visited S Hospital in Gwangju from July 2017 to August 2018. Based on the data obtained from a semi-quantitative food frequency questionnaire (SQ-FFQ), the researcher classified the 112 intake foods of the subjects into 20 food groups and analyzed the association between a food group and cognitive function. From the results it was found that intake by adults over 50 years of ≤2 times/day of cooked white rice (p ≤ 0.005) compared with intake of ≥3 times/day, cooked rice with other grains and legumes (p = 0.015), certain fruits (p < 0.05), milk (low fat and normal) (p = 0.044), liquid-type yogurt (p = 0.019), and curd-type yogurt (p = 0.015) were associated with reduced prevalence of mild cognitive impairment. In addition, this study suggested reducing medications taken by managing the accompanying chronic diseases, maintaining more than 3 hours of sleep time, drinking of adequate alcohol, and balancing lifestyles to alleviate depression. Recently, there has been growing interest in the study of dietary patterns for demonstrating the mixed effects of all the foods in individual diets. Dietary pattern analysis identifies the complex interactions between nutrients and foods, and it has the advantage of explaining how foods ingested by an actual subject are combined. Therefore, dietary pattern research is considered to be a valid method for disease-related investigations that considers the interaction and synergy of nutrients in food. In Study 2, factor analysis was used to analyze the effects of overall food intake trends, such as dietary patterns, on disease rather than the effects of a single food group. From the results of analyzing the association between dietary pattern and cognitive function, we found that a beverage and nuts pattern with a high intake frequency of beverages, nuts, sweet foods, fruits, rice-cake, coffee, tea, and bread had a statistically significant association with the prevalence of mild cognitive impairment (3rd vs. 1st tertile, adjusted odds ratio [AOR], 0.333; 95 % confidence interval [CI], 0.133-0.831; p <0.05).
Subsequent study reflect the limitations of previous studies and utilize the data of the Korean Genome and Epidemiology Study at the Korea Centers for Disease Control and Prevention, a large-scale study of Koreans. In Study 3, we analyzed the dietary patterns of Korean adults by factor analysis and analyzed the effects of the extracted dietary patterns on circulatory disease. We used principal component analysis based on dietary intake data from Koreans aged 40-69 to derive a vegetable and seafood (men)/soup and stew (women) pattern, sweet foods and breads and rice-cake pattern, and multigrain rice and cooked white rice pattern. However, the food groups that made up the dietary pattern were highly varied that it was difficult to define the effects on the disease consistently. Therefore, large-scale follow-up studies using a scientific research approach were performed to determine the dietary factors of diseases that had a social burden and high prevalence.
Rapid economic development and westernized Korean diets are considered to be the main causes of high chronic disease prevalence and Korean mortality. According to the 2013-2015 KNHANES data, the prevalence of metabolic syndrome in men aged 19-49, the main economically active population, was 20.3%, and the prevalence in women was approximately 7.3%. In the following Study 4, the 2016 KNHANES data were used to analyze various working characteristics and health-related factors affecting metabolic syndrome of 19-64 year old workers, who are leading the economic growth of Korea. In summary, the prevalence of metabolic syndrome among men was higher than that of women (28.3% in men and 16.9% in women), and men day workers were statistically significantly associated with increased metabolic syndrome compared with shift workers (adjusted odds ratio, 1.726; 95 % confidence interval, 1.077-2.765). In addition, the sleeping hour of women was found to be associated with metabolic syndrome (≥8 vs. <6 hours/day, adjusted odds ratio, 2.133; 95 % confidence interval, 1.041-4.368). Weekly breakfast intake was found to reduce metabolic syndrome prevalence in both men and women (5-7 times vs. <1 time frequency of breakfasts/week, adjusted odds ratio, 0.593; 95 % confidence interval, 0.372-0.944 in men and 1-2 times vs. <1 time frequency of breakfasts/week, adjusted odds ratio, 0.393; 95 % confidence interval, 0.161-0.962 in women). Based on the results of this study, we suggest the continual provision of nutrition education for appropriate drinking, control of sleep hour, and increase of the breakfast intake rate; and also that a national health promotion plan is necessary for individuals to practice a healthy dietary life.
Taken together, these findings suggest that dietary and health-related factors are associated with cognitive function and the development of cardiovascular disease. Providing dietary guidelines to individuals in a variety of circumstances requires scientific validation of these conclusions. In addition, sensitive and useful biochemical indicators are needed when evaluating the effects of diet on disease, and further studies should be conducted to develop such biochemical indicators.
In future, we propose to conduct clinical interventional studies of various food groups and dietary patterns for long-term large-scale research based on the results obtained from quantitative data.
최근 노인인구가 증가함에 따라 사회적 관심이 높아지고 있는 인지장애와 심·혈관계질환과 같은 만성질환의 발생에 식생활이 밀접한 관계가 있다고 보고되고 있다. 선행연구에서 경도인지장애는 연령이 증가함에 따라 나타나는 정상적인 인지 손상과 알츠하이머 치매 진행의 중간 단계로 정의되며, 질병의 진행을 가속화시키는 위험요인을 관리하기 위한 Practical guidelines의 수립이 지속적으로 요구되고 있다. 이에 따라 최근에 Mediterranean diet와 MIND diet와 같은 식품 섭취가 기억 및 인지기능에 미치는 영향을 분석하는 연구에 관심이 증가하고 있다. 하지만, 질병 예방 및 관리를 위한 지침을 마련하기에는 부족한 실정이다. 그러므로 질병에 영향을 미치는 것으로 알려져 있는 생물학적 특성, 생활양식, 사회경제적 여건을 반영하여 대상자의 식품 섭취에서 나타나는 상당한 변이를 많은 인구 집단에서 정량화 하는 것이 필요하다.
연구 1에서는 2017년 7월부터 2018년 8월까지 광주광역시 S 종합병원에 내원한 50세 이상 276명의 참여자를 대상으로 112항목의 반정량 식품 섭취 빈도 조사를 실시하고, 이를 관련 선행연구 자료를 참조하여 20개 식품군으로 분류하여 각 식품군의 섭취 빈도와 인지기능과의 연관성을 분석하였다. 그 결과, 50세 이상 성인에서, ≥3 times/day에 비해 <1 time/day의 cooked white rice (p < 0.001) 섭취, cooked rice with other grains and legumes (p = 0.015), fruits (p < 0.05), milk (low fat and normal) (p = 0.044), liquid-type yogurt (p = 0.019)와 curd-type yogurt (p = 0.015)의 적절한 섭취가 경도인지장애의 유병률 감소와 연관되어 있다고 확인되었다. 이와 함께, 본 연구를 통해 동반된 만성질환의 관리를 통해 복용하는 약물을 줄이고, 3시간 이상의 수면 시간 유지, 적절한 알코올 섭취, 우울증상을 경감하기 위한 생활 습관의 균형적 조절이 제안되었다.
질병과의 관련 요인을 평가하기 위해 활용한 식사요인 분석은 앞서 연구한 방법과 같이 식품, 식품군 또는 계산된 영양소 섭취를 노출요인으로서 사용하여 실시되는 것이 일반적이었다. 최근에는 사람의 식단에서 섭취한 모든 식품들의 혼합된 효과를 증명하기 위한 식사패턴 연구에 대한 관심이 커지고 있다. 식사패턴 분석은 영양소와 식품들 간의 복잡한 상호작용을 규명하며, 실제 대상자가 섭취한 식품들이 합쳐지는 방법들을 설명하는 장점이 있다. 연구 2에서는 앞서 수행된 단일 식품군 섭취와 질병과의 연관성 규명 연구에서 전체적인 섭취 식품의 식사패턴을 분석하였고, 이를 위해 표준 다변량 통계적 방법을 활용한 요인분석을 사용하였다. 식사 섭취 조사 방법으로 반정량 식품 섭취 빈도 조사 외에 영양소 섭취 분석을 위한 1일의 24-hour recall method 방법을 이용하였다. 식사패턴과 인지기능의 연관성을 분석한 결과, beverages, nuts, sweet foods, fruits, rice-cake, coffee, tea, bread 의 섭취 빈도가 높은 beverage and nuts pattern이 본 연구 성인 대상자에서 경도인지장애 유병과 유의적 연관성이 있음을 확인하였다 (3th vs. 1st tertile, adjusted odds ratio [AOR], 0.333; 95% confidence interval [CI], 0.133-0.831; p-value <0.05). 그러나 본 연구는 병원에 내원한 대상자로부터 조사된 연구로 한국인 50세 이상 성인에게 일반화할 수 없으며, 식사패턴을 추출하는데 활용된 요인분석이 식품을 분류하는 데 주관적 방법이 포함되어있다는 연구 한계점을 보였다.
이어진 연구 3에서는 앞선 연구들의 한계점을 수용하여 한국인을 대표하는 대상자의 순환기계 질환에 영향을 미치는 식사패턴의 도출을 위해 질병관리본부의 한국인유전체역학조사 자료를 활용하였고 얻어진 식사섭취 조사 자료로 요인분석을 활용하였다. 주성분분석으로 vegetable and seafood (men)/soup and stew (women) pattern, sweet foods and breads and rice-cake pattern, multigrain rice and cooked white rice pattern의 식사패턴이 도출되었다. 하지만 식사패턴을 구성하는 식품군이 다양하여 질환에 미치는 영향을 일관적으로 규정짓기 어렵고, 연구 대상자에게 고려되는 질환의 위험인자, 식생활 문화, 추적 기간과 분석 방법이 가변적인 이유로 본 연구에서 도출된 식사패턴이 순환기계 질환에 미치는 영향은 유의적이지 않았다.
급속한 경제 발전과 서구화된 식습관의 영향을 받아 한국인의 식생활은 변화하고 있고, 이로 인해 만성질환이 한국인 사망률의 주요인으로 여겨지고 있다. 2013-2015년 국민건강영양조사 자료에 따르면, 주된 경제 활동 인구인 19-49세 남성 대상자의 대사증후군 유병률이 20.3%, 60세 이상 여성의 경우에는 40% 이상을 나타내었다고 보고하였다. 이어진 연구 4에서는 2016년 국민건강영양조사 자료를 활용하여 한국의 경제 성장을 이끌고 있는 19-64세 근로자의 대사증후군에 영향을 미치는 다양한 근무 특성 및 건강관련요인을 분석하고자 하였다. 결과적으로, 남성 근로자의 대사증후군 유병률이 여성보다 높았으며 (28.3% in men and 16.9% in women), 남성 주간 근로자가 교대 근무자에 비해 대사증후군 발병 상승과 유의적 연관성을 나타내었다 (AOR, 1.726; 95% CI, 1.077-2.765). 또한, 여성의 수면 시간이 대사증후군 발병에 연관성이 있음이 확인되었다 (≥8 vs. <6 hours/day, AOR, 2.133; 95 % CI, 1.041-4.368). 주당 아침 식사 섭취 빈도는 남성과 여성 모두에서 대사증후군 발병을 감소시키는 것으로 밝혀졌다 (5-7 times vs. <1 time frequency of breakfasts/week, AOR, 0.593; 95% CI, 0.372-0.944 in men and 1-2 times vs. <1 time frequency of breakfasts/week, AOR, 0.393; 95% CI, 0.161-0.962 in women).
위의 연구 결과들을 종합해보면, 건강관련 요인과 식생활 환경이 인지기능과 대사증후군 위험에 관련성이 있다고 판단된다. 이런 결과들을 다양한 환경에 처한 대상자에게 식사 지침으로 마련하기 위해서는 과학적 검증이 필요하다. 검증 절차에는 식사요인들은 잘 흡수되지 않거나 빠르게 대사 혹은 배설되고, 혈중 항상성 유지 기전에 따라 조절되는 특성이 고려되어야 하며, 질병에 대한 식사의 평가에 민감하고 유용한 생화학지표의 개발이 중요하다. 추후, 장기간-대규모 대상자에게 정량적 자료를 토대로 한 다양한 식품군과 식사패턴의 중재시험 수행을 제안한다.
