The role of advanced directive has been on the rise, with a controversial view about meaningless medical care for life prolongation induced by development in medical technology. The older populations are predicted to be main subjects of advanced directives, but there was little research on senior’s attitudes toward advanced directives in Korean society.
In this study, we investigated 1) Korean senior’s attitudes toward advanced directives, 2) senior’s tendency to accept advanced directives, and 3) which factors influence attitudes toward advanced directives.
Survey for this study was performed from Sep 7th to 30th of September, 2011. Permission for data collection from two senior welfare centers located in ‘S’ city, and approval from Institutional Review Board of College of Nursing, Seoul National University were obtained before the survey. Total 180 subjects, who met the study criteria, reply to structured questionnaire with individual one-on-one talk.
Advanced Directives Attitude Survey (ADAS) was performed according to the reported tool (Nolan & Bruder, 1997; 2003) with an authorized translation into Korean. The reliability of tool was Cronbach’s alpha 0.74 in the author, and 0.79 in this study. The Data were expressed as the mean ± standard error of mean (S.E.M) and analyzed for statistical significance using one way analysis of variance (ANOVA) followed by Scheffe’s test as a post hoc test and a P value < 0.05 was considered significant.
In this study, subjects generally expressed positive attitudes toward advanced directives, including some subcategories as ‘opportunity for treatment choices’, ‘impact of advance directives on treatment’ and ‘impact of advanced directives on the family’. However, positive tendency in the self-diagnosis disturbed ‘perception of illness’, as a subcategory of advanced directives. In addition, time to have advanced directives was desired to be a beginning stage of illness, not a healthy stage.
Second, age, level of education, monthly income had a significant influence on attitudes toward advanced directives. But, there was no significant difference in attitudes toward advanced directives by socio-demographics characteristics such as sex, religion, marital status, and public insurance type.
Third, number(s) of medical service use in one month, medical expense in one month, acquaintance’s ICU hospitalization within one year, experience of acquaintance’s death within one year, or intension to make advanced directives had a significant relationship with positive attitudes toward advanced directives. However, among health related characteristics, number(s) of chronic illness, experience of hospitalization within five years, ICU hospitalization experience within five years, or perception of health status had no significant relationship with attitudes toward advanced directives.
In this study, Korean senior had a positive attitude toward advanced directives, in spite of their blindness to the advanced directives with necessity to explain in detail. In addition, there is a controversy about factors influencing subject’s attitude toward advanced directives, suggesting that thoughtful approaches with respecting subject’s view of life and values needs to be considered, not only limited on life prolongation.
These comprehensions from present study may provide further insights to derive positive attitudes toward advanced directives from subjects, and help senior to exert active rights in the end of life.
최근 의료기술의 발달로 인간 생명을 연장하는 기술은 나날이 발전하고 있지만 무의미한 연명치료를 행하는 것이 과연 인간으로서의 존엄성을 지킬 수 있는 것인가에 대한 목소리가 높아지고 있는 가운데 사전의사결정에 대한 논의가 이루어지고 있다. 사전의사결정의 대상자는 주로 노인이 될 것이라고 예상되지만 노인을 대상으로 사전의사결정에 대한 태도를 알아 본 연구는 우리나라에서 현재 거의 이루어진 바가 없다.
따라서 본 연구는 노인의 사전의사결정에 대한 태도를 파악하여 우리나라 노인이 사전의사결정을 수용할 준비가 어느 선 까지 와 있는지, 사전의사결정 태도에 영향을 주는 관련 요인은 무엇이 있는지 확인하고자 이루어졌다.
본 연구의 자료수집은 2011년 9월 7일부터 30일 동안 이루어졌다. 자료수집 전 S시 소재 2개 노인 종합 복지관의 자료수집 허락을 받고, 서울대학교 간호대학 연구대상자 보호 심의위원회의 승인을 받은 뒤, 선정과정에 적합한 대상자와 구조화된 설문지로 일대 일 개별면담을 통해 이루어졌으며 총 180명이 설문을 완료하였다.
대상자의 사전의사결정에 대한 태도를 측정하기 위해 Nolan & Bruder(1997)가 만든 Advance Directive Attitude Survey(ADAS)를 동일 저자가 2003년 수정•보완하여 개정한 것을 번역과 역 번역의 과정을 거쳐 사용하였으며, 도구의 신뢰도는 도구 개발자의 연구에서 Cronbach's alpha가 .74였으며 본 연구에서는 .79 였다. 수집된 자료는 연구 목적에 따라 빈도와 백분율, 평균과 표준편차, t-test, one-way ANOVA로 분석하였다. one way ANOVA의 검정에서 유의한 결과는 Scheffe’s test를 이용하여 사후 분석하였다.
본 연구의 결과는 다음과 같다.
첫 째, 본 연구에 참여한 대상자들은 대체적으로 사전의사결정에 대해 긍정적인 태도를 나타냈으며, 사전의사결정 작성에 동의하는 사람은 61.1%였다. 사전의사결정의 하부 항목 중 치료선택의 기회, 사전의사결정이 가족에게 미치는 영향, 사전의사결정이 치료에 미치는 영향에서 모두 긍정적인 태도를 보였다. 반면, 질병의 인식의 경우 사전의사결정을 하기에는 본인의 건강상태를 긍정적으로 자각하는 경우가 많아 사전의사결정에 대한 태도에 긍정적인 영향을 주진 못하였다. 그리고 사전의사결정을 행하는 시점은 완전히 건강한 단계가 아닌 초기 질병단계에게서 행하여 지길 바랐다.
둘 째, 대상자의 인구사회학적 특성 중 연령, 교육수준, 소득수준이 대상자의 사전의사결정에 대한 태도에 유의한 영향을 준 것으로 나타났다. 하지만 성별, 종교, 배우자의 유무, 건강보험 형태는 사전의사결정의 태도에 유의한 영향을 미치지 않는 것으로 조사되었다
셋 째, 대상자의 사전의사결정에 대한 태도에 긍정적인 측면에서 유의한 영향을 주는 건강관련 특성은 지난 한 달 동안 방문한 의료기관의 수, 한 달 동안 지출한 의료비, 지난 1년 동안 지인의 중환자실 입실과 사망에 대한 경험, 사전의사결정에 대한 작성의사 여부였다. 반면 본인이 현재 갖고 있는 만성질환의 수, 자신이 병원에 입원을 했거나 중환자실에 입실했던 경험 유무, 건강상태에 대한 자각 정도는 사전의사결정에 대한 태도와 유의한 관계를 보이지 않는 것으로 나타났다.
이상의 연구 결과, 우리나라 노인들은 사전의사결정에 대해 긍정적인 태도를 갖는 것으로 나타났다. 그러나 사전의사결정에 영향을 주는 대상자의 특성이 선행연구와 상이하게 나온 것을 주목해야 하며, 이는 추후 노인을 대상으로 사전의사결정에 대하여 접근 할 때 단순히 연명치료 유지나 거부의 수단으로서의 접근이 아니라 죽음을 맞이하는 대상자의 인생관과 철학을 이해하는 좀 더 깊이 있는 접근이 필요함을 시사할 수 있겠다
