배경: 요로감염은 전 연령층에서 흔히 발생하는 질환으로 재발이 쉽고 적절하게 치료하지 않으면 심한 신 손상을 초래하게 되므로, 적절한 치료와 추적 관찰을 통해 신 손상을 예방하여야 한다. 소변 배양 검사는 올바른 항균제 선택과 효과적인 치료에 도움이 되며, 또한 원인균의 분리 빈도와 항균제 감수성 결과의 분석으로 경험적 항균제 치료에 도움이 된다.
재료 및 방법: 2006년 1월부터 2015년 12월 까지 10년간 전북대학교병원 진단검사의학과 미생물 검사실에 소변 배양 검사가 의뢰된 검체
122,485건을 대상으로 하였으며, Vitek2 system (bioMerieux, U.S.)을 이용하여 균주 동정 및 항균제 감수성검사를 하였다. 이를 통해 주요 균종의 분리율과 항균제 감수성검사의 변화에 대한 추이를 분석 하였다.
결과: 임상미생물 검사실에 2006년부터 2015년까지 10년간 의뢰 된 소변 배양 검사 122,485건 중 총 30,454건(24.9%)에서 배양 양성이었다. 연령별과 성별 소변 배양 검사의 양성율은 1세미만에서 32.1%로 남자 75.5%,여자 24.5%이며, 1-18세는 19.5%로 남자 60.2%, 여자 39.8%, 19-64세에서 21.2%로 남자 42.6%, 여자 57.4%, 65세 이상에서는 28.9%로 남자 38.6%, 여자 61.3%이었다.
소변 배양 검사에서 분리된 균종으로 그람 음성균이 61.3%이었으며, 그람 양성균이 25.3%, 진균이 13.4%이었다. 주요 원인균으로 E. coli 가 33.4%로 가장 많았으며, E. faecalis가 9.4%, E. faecium 7.8%, K. pneumoniae 6.7%, P. aeruginosa 5.3%의 순이었다.
항균제 감수성 결과는 E. coli의 경우 imipenem, meropenem, amikacin에 대하여 90%이상의 높은 감수성 결과를 나타냈다. 반면 cefepime, cefotaxime, ceftazidime, piperacillin-tazopbactam은 10년간 비교하였을 때 감소하는 결과를 보였다.
E. faecalis는 vancomycin, imipenem, nitrofurantoin, linezolid에 높은 감수성 결과를 보였고, lovoflocacin, norfloxacin에 대한 감수성은 감소되었다.
결론: 이 연구를 통해 요로감염의 원인균은 그람 음성균이 가장 많이 분리되었으며, 다음으로는 그람 양성균, 진균의 순이었고, 균종별로는 E. coli, E. faecalis, E. faecium 순이었다. 또한 각 균종의 분리빈도 및 항균제 감수성에도 많은 변화를 보이고 있어 항균제 선택에 어려움이 있을 것으로 생각 되어 진다. 따라서 지속적인 요로감염 원인균의 분리 추이 및 향균제 감수성에 대한 모니터링이 필요하며, 항균제 내성균의 발생을 피하기 위한 새로운 항균제를 개발하고, 무분별한 항균제의 선택이나 항균제 남용을 피해야 할 것으로 생각된다.
Background: Urinary tract infection (UTI) is a common disease of all ages, and frequently recurred. It could cause serious renal damage without proper management. Urine culture is crucial for the choice of antimicrobial agents for the effective treatment. Also, for the treatment of empirical antimicrobial agents, analysis of the frequency of isolating causative bacteria and antimicrobial susceptibility results are helpful.
Materials and methods: A total 122,485 samples submitted to the microbiological laboratory of the Department of Laboratory Medicine, Chonbuk National University Hospital from January 2006 to December 2015 were enrolled. All the identification and antimicrobial susceptibility tests were done using the Vitek2 system (bioMerieux, U.S.). Analysis of the prevalence of the isolated species and the trends of antimicrobial susceptibilities were performed.
Results: Out of 122,485 urine cultures from 2006 to 2015 in clinical microbiology laboratories, 30,454 (24.9%) were culture-positive cases. For under 1-year-old, the positive rate of urine culture was 32.1%, 75.5% for male and 24.5% for female. For 1-18 years old, the positive rate was 19.5%, 60.2% for male and 39.8% for female. For 19-64 years old, the rate was 21.2%, 42.6% for male and 57.4% for female. For over 65 years old, the positive rate was 28.9%, 38.6% for male and 61.3% for female.
The urine culture showed 61.3% of gram-negative bacteria, 25.3% of gram-positive bacteria and 13.4% of fungi. The major pathogens were E. faecium, E. faecium, K. pneumoniae and P. aeruginosa in 33.4%, 9.4%, 7.8%, and 5.3%, respectively.
Antimicrobial susceptibility results showed more than 90% susceptibilities to imipenem, meropenem and amikacin in E. coli. On the other hand, the susceptibilities of cefepime, cefotaxime, ceftazidime, and piperacillin-tazopbactam were decreased in 10-year trend.
E. faecalis showed highly susceptible to vancomycin, imipenem, nitrofurantoin and linezolid, and sesceptibilities of levoflocacin and norfloxacin were decreased.
Conclusion: In this study, gram-negative bacteria was most frequently isolated as causative organisms of urinary tract infections, followed by gram-positive bacteria and fungi. By species, the most frequently isolated bacteria were E. coli, followed by E. faecalis and E. faecium. In addition, the frequencies of isolation of each organism and antimicrobial susceptibilities of each organism were changed, so it may be difficult to choose the empirical antimicrobial agent. Therefore, it is necessary to monitor the trends of isolation of urinary tract infection pathogens and susceptibility to antibacterial agents persistently. Furthermore, it is necessary to develop a new antimicrobial agent to avoid the occurrence of antimicrobial resistant bacteria, and to avoid the indiscriminate choice of antimicrobial agents and abuse of antimicrobial agents.
