Developmental dyslexia diagnosed in childhood persists throughout one's life. Those with developmental dyslexia have difficulty acquiring reading and related cognitive skills. However, research on developmental dyslexia has predominantly focused on children. This study aimed to evaluate the neurobiological characteristics of adults with developmental dyslexia using a brain connectivity analysis and meta-analysis to confirm how the brains of adults with developmental dyslexia support reading.
In Study 1, we investigated the white matter pathways (AF, ILF, IFOF, FAT) associated with reading in young adults without developmental dyslexia. This study aimed to investigate the characteristics of white matter pathways related to reading and examine whether the development of homogeneous white matter pathways is necessary for successful reading. Twenty typical adult readers without developmental dyslexia participated in this study. The results revealed leftward lateralization in the number of AF and ILF fibers. IFOF revealed rightward lateralization of fractional anisotropy. However, there was substantial inter-individual variability. This study established baselines for white matter pathways in the brains of typically developing Korean individuals without developmental dyslexia. These findings imply that the white matter pathways of people who have attained normal reading skills are heterogeneous.
In Study 2, we investigated the structure and functional brain connectivity of adults with developmental dyslexia. The purpose of this study was to examine whether adults with developmental dyslexia, who have different types of reading impairments, exhibit neurological heterogeneity. The DTI tractography results did not reveal any significant differences in white matter pathways between the age-matched controls and the dyslexic cases; however, the RSFC results showed distinct abnormalities in different neural networks. In particular, cases with lower word decoding abilities showed decreased functional connectivity in the visual network and sensorimotor network. Whereas, cases with lower reading fluency and reading comprehension showed decreased functional connectivity in the dorsal attention and salience networks. These results suggest that the behavioral heterogeneity of dyslexic adults can be neurologically distinguished and that the differences are due to functional defects, not structural ones.
Study 3 focused on identifying key biomarkers in the population with developmental dyslexia by investigating their homogeneity. To achieve this, a meta-analysis was conducted on 24 studies, distinguishing between studies involving children and adults with developmental dyslexia. The results revealed that both children and adults with developmental dyslexia exhibit similar deficits in the activation of the fusiform gyrus, and compensatory activity was observed only in adults. Dyslexic adults demonstrated significant hyperactivation in cortical subregions near the basal ganglia during reading tasks. These findings suggest that the fusiform gyrus, where the visual word form area (VWFA) is located, serves as a crucial biomarker for developmental dyslexia, indicating a potential failure of successful specialization of the VWFA starting from childhood.
As a result, reading interventions for individuals with developmental dyslexia should emphasize the ability to analyze the visual characteristics of letters and recognize words. Developmental dyslexia has been accounted for by phonological theory. However, the results of this study suggest that in order to establish reading interventions in the population, it is necessary to strengthen the understanding of the connectivity between brain functions constituting the reading system, not just a phonological approach.
발달성 난독증이 일생에 걸쳐 개인의 삶에 영향을 미친다는 결과가 반복적으로 보고되고 있으나, 해당 인구에 대한 조사는 여전히 아동에 치중되어 있다. 읽기 기술의 결함으로 대학 교육과정에 어려움을 겪고 있는 성인 발달성 난독증을 설명하는 문헌은 드물며, 특히 구체적인 근거로 작용할 수 있는 신경영상연구는 더욱 부족한 실정이다. 따라서 본 연구는 뇌연결성 분석과 메타분석을 통해 성인 발달성 난독증의 뇌가 어떻게 읽기를 지원하는지 확인하고자 하였으며, 이를 통해 해당 인구의 중재접근법 확립에 요구되는 실제적인 근거를 제시하고자 하였다.
연구 1에서는 성인 발달성 난독증의 뇌를 살펴보기에 앞서, 전형적인 읽기 발달과정을 거친 한국 청년층의 뇌연결성을 조사하였다. 이를 위해 읽기를 지원하는 백질경로인 궁상속(arcuate fasciculus; 이하 AF), 하종속(inferior longitudinal fasciculus; 이하 ILF), 하전두후두속(inferior fronto-occipital fasciculus; 이하 IFOF)과 frontal aslant tract (이하 FAT)을 분석하였으며, 각 백질경로에 대해 이방성(fractional anisotropy; 이하 FA) 및 섬유 수를 확인하였다. 그 결과 AF와 ILF의 경우 좌측의 섬유 수가 우세하였으며, IFOF의 경우 우측의 FA가 우세하였다. 개별적으로 살펴본 결과에서 개인 간 상당한 편차가 관찰되었으나, 이들의 읽기 능력은 정상 범위에 해당하였다. 이러한 결과는 한국 정상 성인의 백질경로에 대한 기준을 제시함과 동시에 성공적인 읽기 기술의 획득을 위해 동질한 백질경로의 발달이 필수적으로 요구되는지에 대한 의문을 제기하였다.
따라서 연구 2에서는 성인 발달성 난독증의 백질경로를 정상 성인과 비교하여 이들의 구조적 연결성이 정상 성인에게 관찰된 변동성 내에 위치하는지 확인하였다. 또한신경네트워크에 대한 기능적 연결성 분석을 통해 이질적인 읽기 특성이 확인된 성인 발달성 난독증 사례의 신경학적 이상을 뇌연결성 측면에서 규명하고자 하였다. 그 결과 구조적 연결성 분석에서 정상 성인과 유의한 차이를 나타내는 백질경로는 관찰되지 않았으며, 기능적 연결성 분석에서 두 사례 간 서로 다른 신경네트워크의 이상이 관찰되었다. 단어해독에 결함이 있던 사례는 시각네트워크(visual network; 이하 VN)와 감각운동네트워크(sensorimotor network; 이하 SMN)에서 낮은 기능적 연결성을 나타내었다. 반면 단어해독에는 문제가 없으나 읽기유창성과 읽기이해에 문제가 있었던 사례의 경우 등쪽주의네트워크(dorsalattention network; 이하 DAN), 현저성네트워크(salience network; 이하 SN)에서 낮은 기능적 연결성을 나타내었으나, VN의 기능적 연결성은 비전형적으로 높았다. 이러한 결과를 통해 서로 다른 읽기 특성을 보이는 성인 발달성 난독증 사례의 이질성이 신경학적으로 구분되나 그 차이는 구조적인 것이 아니라 기능적인 결함에 의한 것임을 확인하였다.
연구 3은 발달성 난독증 인구의 핵심 바이오마커를 찾아내기 위해 이들의 동질성에 주목하였다. 이를 위해 현시점까지 수행된 발달성 난독증의 신경 영상 연구를 아동과 성인으로 구분하여 24개 문헌에 대한 메타분석을 수행하였다. 그 결과 발달성 난독증 아동과 성인이 방추상회(fusiform gyrus)의 활성화에 동일한 결함을 갖고 있으며, 이를 극복하기 위한 보상적 활동은 성인에게서만 관찰됨을 확인하였다. 난독증 성인은 읽기 과제 시 기저핵 부근의 피질하영역에서 유의미한 과활성화를 나타내었다. 이러한 결과는 시각단어형태 영역(visual word form area; 이하 VWFA)이 위치한 방추상회가 발달성 난독증 인구를 설명하는 핵심 바이오마커이며, 발달성 난독증 집단이 이미 아동기 때부터 VWFA의 성공적인 문자 특정화(specialization)에 실패했을 가능성을 시사하였다.
본 연구의 결과를 종합했을 때, 성인 발달성 난독증의 읽기 중재 시 글자의 시각적 특질을 분석하고 이를 재인하는 능력이 더욱 강조될 필요가 있다. 발달성 난독증의 뇌연결성 분석을 통해 이질적인 신경네트워크의 결함을 확인하였으나, 두 사례가 공통적으로 이상을 보인 네트워크는 초기 시각처리과정을 담당하는 VN이었다. 또한 메타분석 결과는 시각단어재인에 관여하는 VWFA에 해당 인구의 핵심적인 결함이 있음을 확인하였다. 언어병리학 측면에서 발달성 난독증은 음운 이론에 입각하여 설명되어왔다. 그러나 본 연구의 결과는 해당 인구의 읽기 중재 확립을 위해 단순 음운론적 접근에 그치지 않고 읽기 시스템을 구성하는 ‘뇌 기능 간 연결성’을 강화시켜야 한다는 점을 시사한다.
