연구목적: 본 연구의 목적은 중환자실 간호사의 다제내성균 감염관리 임파워먼트, 다제내성균 감염관리 환경안전성에 대한 인식이 다제내성균 감염관리 수행자신감에 영향을 미치는가를 확인함으로써 효율적인 다제내성균 감염관리 중재 프로그램 기초 자료로 활용하고자 한다.
연구방법: 본 연구는 서술적 조사연구로 대상자는 B광역시에 소재한 2개 종합병원(H병원, P병원)에 근무하는 내과계, 외과계 중환자실 간호사 중 연구 목적을 이해하고 이에 동의한 간호사 150명을 대상으로 자료를 수집하였으며, 총 141부의 자료가 분석되었다. 본 연구에서 중환자실 간호사의 다제내성균 감염관리 임파워먼트, 다제내성균 감염관리 환경안전성에 대한 인식 및 다제내성균 감염관리 수행자신감에 대한 도구는 설문지를 사용하였고, 자료 수집은 2020년 9월 14일부터 2020년 9월 28일까지 실시하였다. 수집된 자료는 IBM SPSS window 25.0 program을 이용하여 기술통계, t-test, one-way ANOVA, Pearson’s correlation coefficients, Hierarchical multiple regression으로 분석하였다.
연구결과: 본 연구의 결과는 다음과 같다.
1. 대상자의 일반적 특성 중 연령은 26∼30세가 70명(49.6%)으로 가장 많았으며, 평균은 29.06세(±3.83)이었다. 중환자실 근무경력은 1∼3년 미만 43명(30.5%)으로 가장 많았으며, 평균은 3.51년(±2.37)이었다. 성별은 남 28명(19.9%), 여 113명(80.1%)이었다. 학력은 전문학사 23명(16.3%), 학사 114명(80.9%). 석사 이상 4명(2.8%)이었다. 중환자실 근무만족도는 ‘전혀 만족하지 않는다’ 4명(2.8%), ‘만족하지 않는다’ 9명(6.4%), ‘보통이다’ 70명(49.6%), ‘만족한다’ 50명(35.5%), ‘매우 만족한다’ 8명(5.7%)이었다. 근무 시 담당 환자 수는 3명 116명(82.3%)으로 가장 많았다. 작년 한 해 다제내성균 환자를 간호한 경험은 ‘예’ 136명(96.5%), ‘아니요’ 5명(3.5%)이었다. 근무병원의 다제내성균 감염관리지침 여부는 모두 있다고 응답하였다. 근무병원의 다제내성균 감염관리지침 수행 시 적용하는데 만족 여부는 ‘예’ 107명(76.4%), ‘아니요’ 33명(23.6%)이었다. 최근 1년간 다제내성균 감염관리에 대한 교육경험은 ‘있다’ 95명(67.4%), ‘없다’ 46명(32.6%)이었다.
2. 대상자의 다제내성균 감염관리 임파워먼트 점수는 60점 만점에 평균 44.06±6.57점 이었고, 다제내성균 감염관리 환경안전성에 대한 인식 점수는 28점 만점에 평균 22.89±2.38점 이었고, 다제내성균 감염관리 수행자신감 점수는 160점 만점에 평균 134.51±17.69점 이었다.
3. 대상자의 다제내성균 감염관리 임파워먼트는 일반적 특성에서 중환자실 근무만족도(F=3.70, p=.027)와 다제내성균 감염관리지침 적용 만족 여부(t=2.43, p=.016) 에서 유의한 차이가 있었다. 다제내성균 감염관리 환경안전성에 대한 인식은 직위(F=3.14, p=.046), 중환자실 근무만족도(F=15.96, p<.001), 다제내성균 감염관리지침 적용 만족 여부(t=3.83, p<.001), 다제내성균 감염관리 교육 여부(t=2.33, p=.023)에서 유의한 차이가 있었다. 다제내성균 감염관리 수행자신감은 직위(F=4.68, p=.011), 중환자실 근무만족도(F=3.96, p=.021)에 유의한 차이가 있었다.
4. 대상자의 다제내성균 감염관리 수행자신감은 다제내성균 감염관리 임파워먼트(r=.46, p< .001)와 다제내성균 감염관리 환경안전성에 대한 인식(r=.37, p<.001)에 정적상관이 있었으며, 다제내성균 감염관리 임파워먼트와 다제내성균 감염관리 환경안전성에 대한 인식 간에도 정적상관(r=.33, p<.001)이 있었다.
5. 대상자의 다제내성균 감염관리 수행자신감에 유의하게 영향을 미치는 요인은 다제내성균 감염관리 임파워먼트(β=.30, p<.001), 다제내성균 감염관리 환경안전성에 대한 인식(β=.29, p= .001), 직위(β=.24, p=.001)였고, 설명력은 30.6%이었다(Adj-R2=.306, F=11.28, p<.001).
결론: 본 연구 결과를 종합해 보면, 다제내성균 감염관리 임파워먼트가 높을수록, 다제내성균 감염관리 환경안전성에 대한 인식이 높을수록, 중환자실 간호사의 직위가 높을수록 다제내성균 감염관리 수행자신감이 높았다. 따라서 다제내성균 감염관리를 위한 적절한 환경조성을 기반으로, 직위에 따른 임파워먼트를 강조한 교육프로그램과 실무지침을 통해 다제내성균 감염관리 수행자신감을 높이는 관리 전략이 필요하다.

Abstract
Factors Influencing Performance Confidence
of Multi-Drug Resistant Organisms(MDRO)s Infection Control among Registered Nurse in Intensive Care Unit

Purpose: The purpose of this study is to confirm whether the empowerment and environmental safety awareness of multi-drug resistant bacteria infection management affects the performance confidence of multi-drug resistant bacteria infection management for intensive care unit nurses, and used as basic data for an effective multi-drug resistant infection control intervention program.
Methods: This study was a descriptive correlation study. Among nurses working at two general hospitals (H Hospital and P Hospital), 150 nurses in intensive care units who understood and agreed to participate in the research, and 141 copies of the data were analyzed. In this study, tools for multi-drug resistant bacteria infection control empowerment, environmental safety awareness and performance confidence of intensive care nurses were used, and data collection was conducted from September 14, 2020 to September 28, 2020. The collected data were analyzed with descriptive statistics, t-test, one-way ANOVA, Pearson’s correlation coefficient ,Hierachical multiple regression using the IBM SPSS window 25.0 progam.
Results: The results of this study are as follows.
1. As for the general characteristics of the subjects, the age of 26∼30 was the most common with 70 persons (49.6%), and the average was 29.06 years (±3.83). The most frequent work experience in the intensive care unit was 43 persons (30.5%) for less than 1 to 3 years, and the average was 3.51 years (±2.37). The gender was 28 male (19.9%) and 113 female (80.1%). The academic background was 23 persons (16.3%) with a College degree, 114 persons (80.9%) with a bachelor's degree and 4 persons (2.8%) with a Master's degree or higher. Work satisfaction in the intensive care unit was “not satisfied at all” with 4 persons (2.8%), “not satisfied” with 9 persons (6.4%), “normal” 70 persons (49.6%), “satisfied” 50 persons (35.5%), “Very satisfied” 8 persons (5.7%). The number of patients in charge when working was the largest with 3 patients 116 persons (82.3%). Last year, 136 persons (96.5%) with ‘Yes’ and 5 persons (3.5%) with ‘No’ were Nursing experience of MDROs patients. They answered that they had all the guidelines for MDROs infection control in the hospital. When implementing the guidelines for MDROs infection control at the hospital, the satisfaction status was 107 persons “Yes” (76.4%) and 33 persons “No” (23.6%). In the past 1 year, the education experience of MDROs infection control was 95 persons with “Yes” (67.4%) and 46 persons with “No” (32.6%).
2. The multi-drug resistant bacteria infection control empowerment score of the subject was an average of 44.06±6.57 points out of 60 points, and the environmental safety awareness score of multi-drug resistant bacteria infection control was an average of 22.89±2.38 points out of 28 points, and the performance confidence score of multi-drug resistant bacteria infection control was an average of 134.51±17.69 out of 160 points.
3. According to the general characteristics of the subjects, there was a significant difference in multi-drug resistant bacteria infection control empowerment in intensive care unit work satisfaction (F=3.70, p=.027) and satisfaction with the application of management guidelines of Multi-drug resistant bacteria infection control (t=2.43, p=.016). Multi-drug resistant bacteria infection control environmental safety awareness is based on position (F=3.14, p=.046), intensive care unit work satisfaction (F=15.96, p<.001), satisfaction with application of management guidelines of Multi-drug resistant bacteria infection control (t=3.83, p<.001), and education experience on Multi-drug resistant bacteria infection control (t=2.33, p=.023) there was a significant difference. Performance confidence of multi-drug resistant bacteria infection control was significantly different in job position (F=4.68, p=.011) and intensive care unit work satisfaction (F=3.96, p=.021).
4. Performance confidence in multi-drug resistant bacteria infection control of the subject was positively correlated with Multi-drug resistant bacteria infection control empowerment (r=.46, p<.001) and Multi-drug resistant bacteria infection control environmental safety awareness (r=.37, p<.001). There was also a positive correlation (r=.33, p<.001) between the Multi-drug resistant bacteria infection control empowerment and Multi-drug resistant bacteria infection control environmental safety awareness.
5. In the hierarchical multiple regression analysis, the factors that significantly affect the subject's in performing confidence of multi-drug resistant infection control were multi-drug resistant bacteria infection control empowerment (β=.30, p<.001), multi-drug resistant bacteria infection control environmental safety awareness (β=.29, p=.001), and job position (β=.24, p=.001), and then explanatory power was 30.6% (Adj-R2 =.306, F=11.28, p<.001).
Conclusion : In summarizing the results of this study, the higher empowerment of the multi-drug resistant bacteria infection control, the higher environmental safety awareness of the multi-drug resistant bacteria infection control, the higher the position of the intensive care unit nurse was Multi-drug resistant bacteria infection control performance confidence higher. Therefore, based on the creation of an appropriate environment for multi-drug resistant bacteria infection control, a management strategy is needed to improve multi-drug resistant bacteria infection management through educational programs and practical guidelines that emphasize Multi-drug resistant bacteria infection control empowerment according to position.
